CONTRACT OF PERSONAL SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This contract made and entered into by and between:

The Civil Aviation Authority of the Philippines (CAAP),

government owned and controlled corporation with
principal office and business address at NAIA Road,
Pasay City, herein represented by Director General
LT GEN WILLIAM K HOTCHKISS AFP (Ret),
and
hereinafter referred to as the FIRST PARTY.
- and MA. VIRGINIA P. EBUENGA, married, of
legal age, with a postal address at
Upper Dicayas, Dipolog City, hereinafter
referred to as the SECOND PARTY.
WITNESSETH
WHEREAS, the Civil Aviation Authority of the Philippines (CAAP) is the
primary government agency responsible for the development of a safe,
dependable, economical and efficient air transportation system, taking into
account the requirements of national security;
WHEREAS, the CAAP needs the services of Airport / Facility Cleaner I in
its desire to keep and maintain its premises clean, orderly and sanitary at all
times;
WHEREAS, the Second Party has offered his/her services to the First Party
and had signified his/her willingness and acceptance to undertake the job to
render janitorial, sanitation, maintenance and related services;
NOW THEREFORE, for and in consideration of the foregoing premises, the
PARTIES hereto have agreed to enter into a CONTRACT OF PERSONAL
SERVICES, subject to the following terms and conditions.
I.

OBLIGATION OF THE SECOND PARTY:

a. Shall report directly to the OIC, Dipolog Airport.
b. Shall perform janitorial, sanitation, maintenance and related services at
CAAP Premises/Facilities.

Shall provide extra service of running errands within and along the
vicinity of the building/facility and other related tasks as warranted and
needed such as moving/transferring office equipment/furniture within
the facility.

OBLIGATION OF THE FIRST PARTY

a.

The FIRST PARTY hereby agrees to give the SECOND PARTY a daily
compensation of Four Hundred Nine Pesos (P409.00), subject to
availability of funds and usual accounting and auditing requirements;

b.

The FIRST PARTY shall provide the SECOND PARTY with Identification Card
upon submission of required documents.
SPECIAL PROVISIONS

a. The Contract of Personal Services shall take effect on 01 July 2016 to 31

December 2016 subject to renewal and termination at the option of the
FIRST PARTY when a) the services are no longer needed b) funds are no
longer available c) his/her performance is below par.

Such services maybe terminated before expiration of the contract if any of

the above conditions in paragraph a/b/c and other valid grounds that may
occur during the stated period.
b. The SECOND PARTY shall submit an accomplishment report every 15 th and
30th of the month anent the execution of his duties and responsibilities and
shall render services at least 40 hours a week.
c. That the SERVICE CONTRACTOR shall not be entitled to any benefits due to
regular and temporary employees.
d. Services under this Contract are not considered government service and
therefore not covered by the CSC Rules and Regulations but covered by the
Commission on Audit (COA) rules.
IN WITNESS WHEREOF, the parties hereby sign this contract this
