Under the Supervision of Sam Smith, LMHC, LCAC, and LFMT: Dual Diagnosis
Programming IOP for Drug and Alcohol addictions.
Patients are diagnosed with a Mental Health Issue along with and addiction
(Dual). Primary is determined at assessment.
Skills acquired during internship: appropriate knowledge of the principles
of growth and development over the life span of the patient. Learn more
concerning the ability to assess patient data relative to age specific needs
and provide care as described in the department's policies and procedures.
Gained knowledge of the progression of the addiction and 12-step recovery
while working with patients with addiction. Developed good Oral, Written
communication skills within the clinical field.
Change Diagnosis if necessary, provides direct clinical services to groups
Assist patients in the completion of their first two steps moving them to AA
or NA meetings
Complete treatment plans, assessments, and orientation of services, and

Organize group sessions allowing members to process with supervised
instructor Sam Smith from 6-8pm and do education sessions from 8-9pm

Presently work in an inpatient setting on acute care unit for adults. Using
motivational interviewing skills, CBT techniques, DBT techniques, to insure that
patient are receiving evidence based treatments, and teaching coping skills and
allowing patient to recognize and understand their triggers. Primary diagnosis is
mental health.
Responsibilities:
Are to complete biopsychosocial assessments, group therapy, one-on-one
sessions, and complete discharge summaries.
Making sure that all therapeutic treatment plans are established and
treatment goals are met while patients are in the inpatient unit at options

Teaching coping skills and allow each patient to acknowledge and learn
that triggers while they're in the inpatient unit

Rehabilitation Teacher for the Blind and Visually impaired
Facilitated workshops and symposiums that were to give information about free
services that Blind and Visually Impaired Services, in five counties offered for the
citizen within the State of Indiana were blind or visually impaired.
Responsibilities
Instructed those who were blind or visually impaired on daily living skills
needed to function continually in their homes and community
surroundings
Taught skills using computers, tape recorders and other adaptive
equipment, as well as instructed on Braille for reading purposes for the
blind
Ordered and trained in the use of all adaptive equipment ordered for the
clients use to assist them to continue skills learned by our services,
equipment to enhance their daily living: CCTV's, magnifiers, cooking
equipment, templates for check writing, and writing purposes as well as
reading material written in Braille for continual practices.
