Work history since graduation with a dentistry degree, listed in chronological order,
including dates, locations, types of employment. Dentistry experience should include
types of practice experience. Explain any voids in the sequence of dates and/or work
experience.
National, provincial, state licenses held listed chronologically including dates
Professional honours and awards, listed chronologically with dates
Research and/or educational grants approved and awarded, with dates (if any)
Professional dental publications, including abstracts and minor publications (if any)
