Evidence/Indicators

- I have entered family conversations and


observed the interaction they have with, and the
influence they have on clients.
- I visited the community diner and saw the
interactions between family members. I spoke with
families and heard stories of their journey and use or
hesitation to use social tools in the community.
I continue to understand that the
presence of family on the mental health floor is not
necessarily indicative of a good or bad family.
Every family is different and deal with crises in
different ways.
- I understand the importance of family and
how they can have a hand in the building up or
tearing down of a person.
I continue to see a large problem with
mental health in the 60 years+ population. Through
conversations I have learned about how much loss
they feel (friends, family, life purpose etc.) and how
this leads to the deterioration of their sense of self.
- I have seen how the combination of low
socioeconomic resources have gotten some clients to
the place they are and how those persisting
conditions prevent them from maintaining progress
made in hospital (ex. Not being able to keep a job
because of issues related to a mental illness, but not
being able to afford medication to control your
mental illness because you cannot keep a job).
I have seen how mental health issues
arise in every demographic and how each person has
a unique experience to how they have come to be in
the place they are.
I use respect in all of my interactions.
Specifically, I have found it necessary to use this with
staff as it has opened the door for me to gain new

the healing of clients!).
I continue to use skills that honour the
client and allow them to guide the interaction so that
it is helpful for the healing they need to have (instead
of me trying to force topics on them hoping it will
illicit healing).
I have begun to be more present and
solely focused on the conversation that I am in. In
this I have begun to be less preoccupied with what I
am going to say next or the location of my hands,
and more attuned with the client and conversation at
hand.
- I ask questions of the staff when I observe
them using certain behaviours or asking certain
questions so I understand the reason behind it. From
there I attempt to be more mindful about using that
technique when appropriate.
I have used activities (board games,
walks, crafts, and cupcake decorating) to build
relationships and start conversations with patients
who may not have been open to me previously.
- When working with a patient that has a
history that I am troubled by, I ensure to think back
to my training and ensure that I reflect on those
feelings and strive to be unbiased in my interactions.
I review patient's charts to understand
possible triggers and the direction their care is going.
This helps me form a plan for how to interact with the
client and hopefully bring about some kind of
meaningful change.
- I have gained an understanding of how to
balance reflexivity during client interactions, while
still being present and not allowing preoccupation
with using the correct communication skills to block
what feels natural.
- I continue to incorporate feedback from
peers, and instructors into my practice. I also use
reflection to better my practice and the way I interact

- I have spoken about coping techniques
with clients, and allowed them to identify things that
they may find helpful in promoting health in
themselves outside of the hospital.
- I continue to attend lab fully prepared
to learn and ensure I am confident with the skill
before performing it in lab.
- I have watched the nurses distribute
oral medications (it was not possible for me to
complete this independently).
- I have used given time in clinical to
practice skills with my clinical instructor (vital signs,
injections etc.).
- I completed vitals and mental
assessments on clients needing that care.
I have always shown up on time and
ready to start work. I ensure my breaks are
completed on time and my peers or clinical leader
know where I am if I am not with the group.
- I arrive in professional attire and ensure
that I have my badge and it is displayed so people
know that I am a student.
I continue to sign off on my performed
skills (rounds, vitals check) to ensure my actions can
be traced back to me should they need to be
confirmed or questioned.
- I maintain a nurse-client relationship with
clients while they are admitted and ensure I maintain
the boundaries that this entails.
- I researched and lead a group discussion
on Peterboroughs mental health services (ex. ACTT,
Four Counties, LYNX, PHOENIX, Fresh Start, Rural peer
to peer support services etc.). In doing this I am
confident I can do the same for any city/ town a client
is based in.
- I have become comfortable asking clients
if they are aware of a certain service or community
support, and with the okay of their nursing, gathering

information on the program for the client.


I have participated in activities
(meditation, yoga) such that I am able to tell the
client about an support option from personal
experience.

1. I excel in forming therapeutic relationships and finding creative ways to bond with clients
(colouring, board games etc.).
2. I have become confident at seeking out learning opportunities and asking questions to improve my
own practice.
3. I think I have refined my skills in obtaining vitals and performing mini mental assessments.
Student Areas for Future Development
1. I will continue to work on my practical skills such as medication distribution and
injections, and seek out opportunities related to maternity-child skills that I did not practice in this
placement.
2. I will become more knowledgeable of medications frequently seen on mental health floors such as
antidepressants.
3. I will continue to work on being truly present and using actively listening in my conversations with
clients. This will require me to further work on balancing reflexivity with being fully present in the
conversation.
