Dear Womens Healthcare Patient: Thank you for being a patient of MSU Women's Healthcare. We are honored to have been selected as the provider of choice for your obstetrical and gynecological needs. Unfortunately, after over 20 years of serving Mid-Michigan, MSU Womens Healthcare practice will formally close on June 30, 2016. The closure is a response to provider retirement and resignations that leave us unable to adequately provide the level of care you expect from MSU HealthTeam. This letter serves to notify you of this decision and to assist your transition to a new provider. It consists of three parts:

Finding a New Provider

When calling to schedule a new patient appointment at one of these practices,
please identify yourself as a MSU Women's Healthcare patient. The front desk staff at each practice will be expecting calls from patients identifying themselves as MSU Womens Healthcare patients seeking to transfer care. Your current provider may also have other suggestions. Ultimately, you will need to find a new gynecologist or obstetrician. Obtaining your medical records To facilitate establishing care with your new provider and for your own files, you will want to obtain your medical records from MSU Womens Healthcare. We have attached the MSU HealthTeam form you need to request a release of your medical records. Your records will be kept for at least 7 years after the date of your last visit. After June 30, 2016, you may obtain your records by contacting MSU HealthTeam at 517.353.3000 or at HealthTeam Medical Records, 804 Service Rd., A109G, East Lansing, MI 48824.
