Instruction for the use of the logbook

Aim of the logbook
The purpose of the logbook is to provide one source of evidence for the specialty
postgraduate curriculum to the desired level of competency required for licensure.
Sections of logbook

Personal Information
Please fill in your personal information required in page 1.
Your personal photo should be attached to the logbook and
you should sign the personal information page.

Clinical Case log

Patient name is not required.
For each case, write the date of the interview.
Make a degree from 1: 3 at the appropriate column indicating your level of
participation in case management.
Each case should be counter signed by your senior pediatrics medical staff. His
signature is the proof of your actual participation.

Procedures Log
You should write down the management of procedures, your level of participation
and the date.
Your supervisor medical staff member should countersign each procedure to
document the event.

Academic Activities
It means attendance of conferences, workshops and lectures.
Any attended activity must be signed by the workshop or conference
organizer/coordinator.

Assessment of logbook activities

Your supervisor medical stuff will assess your logbook monthly for completion and
provide feedback and countersign important activities.
The examination committee of the pediatric department will revise your logbook:
annually and before the final exam.
Notes:
1-Unsatisfactory logbook at the end of training will prevent you from entering the
final exam.
2-If you meet any difficulty; please, contact your medical staff or your specialty
administrator at the pediatric department.
