1. Performed tuning on BFW Control valve. Badly fluctuate.

2. Troubleshooting on FGRU flaring. Instructed by Mr. Ropa to close FGRU Tie-in


valve with S-701 and S-707. Impact to S-707 and S-701 less flaring. Found not
so much impact to the S3-932. Supposed to be flaring at S3-932 will be
bigger. Suspected pilots were off and nobody knows. Maybe a few days in
that condition. After a close observation and experiment, confirmed pilot in
off condition. Pilot re-ignited and flare back to normal.
3. Led about 3 fire drills.
4. eSOSA almost 30.
5. AGT confined space (registered DOSH). Whenever in absence of AGT confined
space, willing to take responsibility even not as part in routine job. E.g. Q4744 cleaning activity, FW pipe replacement project and COGEN.
6. Reviewer for almost 30 JSA in COGEN project.
7. Author for 2 Standing Instructions.

14.POD HSEAT Leader. Led 2 HSEAT walkabouts at GPS (COGEN project site)
together with Mr. Ropi, Civil Team and Ir. Salam. Event has been recorded,
reported, and shared in PGB Homepage. Another 1 at GPK also COGEN project
site.
15.Advisor for Utility EIS and Battery Limit preparation. Monitor, supervise and
support the team in the aspect of material and man power.
16.Shift Superitendant for area 3 (AGRU and Compressor). Succesfully handover
timely with safe condition for mechanical day. Monitoring battery limit activity
closely without fail.

HSE PILLAR
1. POD HSEAT Leader. Led 2 HSEAT walkabouts at GPK & GPS (COGEN project
site) together with Mr. Ropi, Civil Team and Ir. Salam. Event has been
recorded, reported, and shared in PGB Homepage.
2. Representing GPK during DOE Audit. As a result, achieved very good
compliancy on the requirements.
3. Contributes in GPK 4 COGEN project. Reviewed more than 15 JSA as a
total. Also part of the team in reviewing DCS layout during detailed
engineering stage. Personal feedback expressed out clearly on what is
needed and what improvement can be done to help our panel operator
run the system efficiently in the future.
4. Actively respond whenever incident happened during my shift. 3 cases
happened and INF and iHSE report were submitted within 24 hours. 3
cases are GPK 2 lamp post near to EDG collapsed, GRE project CP cable hit
by excavator and the latest one is fire alarm call point cable scratched
during manual excavation for FW GRE pipe project.
5. Actively performed self-audit and walkabout during PRR 4. One of the
focus area is storm water drain (SWD). Personally check SWD point to
ensure it has been covered by wet fire blanket whenever its surrounding,
within 5 meters radius had machines operated nearby.
6. Reviewer for almost 15 JSA in COGEN project early this year mostly on
electrical and instrument work.
FINANCIAL EXCELLENT PILLAR
1. Performing troubleshooting on flaring issue. Checked together with all
panel & field operator the potential of flaring sources. Found that kickback
valve of R-601B is malfunction. After swapping to R-601A, flaring reduced.
2. Led utility team side during M2-751 leak rectification job together with
process team. During execution, instruct & coordinate for equipment
isolation, depressurizing and draining activities. Rectification of line
successful and with that recovery of condensate return improved.
3. Keep ensuring waste heat boiler optimization is always been monitored in
order to reduce aux boiler fuel gas consumption.
PLANT EXCELLENT PILLAR
1. Successful awarded as 2nd Grade Steam Engineer on Dec 13.
2. Led C3/C4 rectification job. Planned for N2 purging strategy, fitting
fabrication, and LEL monitoring. As a result, line successfully safe repaired
and no HSE issue.
3. Advisor for Utility EIS and Battery Limit preparation DURING Pre-PRR 4 4.
Monitor, supervise and support the team in the aspect of knowledge,
material and man power.

4. Appointed as a Shift Superintendent of Area 3. We successfully managed


utilized operation day period and managed to handover for mechanical
day timely.
5. Successfully awarded as IETS competent person on Feb 14.
6. Successfully close ACD gaps on last assessment.
7. Performed tuning on GPP 2 BFW control valve of F2-771 (2FV-7704). Badly
fluctuate about +/- 20 T/hr causes level of boiler also fluctuate. As a result
from the tuning, BFW fluctuation minimized to +/-5 T/hr.
8. AGT confined space (registered DOSH). Whenever in absence of AGT
confined space, willing to take responsibility even not as part in routine
job. E.g. Q4-744 cleaning activity, FW pipe replacement project, COGEN
and GPP 2 Auxiliary boiler.
