KNOW ALL MEN BY THESE PRESENTS:

I, ENRICO ACLADO JATULAN, of legal age, Filipino and resident of B34 L2 Pacific Parkplace
Paliparan I, Dasmarias, Cavite, do hereby NAME, CONSTITUTE and APPOINT, MR. FERNANDO A.
ZACATE, likewise of legal age, Filipino and resident of Brgy. Mara Mara, Sulat, Eastern Samar, to be my
true and lawful ATTORNEY IN FACT for me and in my name and stead and to perform the following
acts and deeds, to wit:

To sign, execute, represent and to file any appropriate case against any and all persons
relative to the incident/accident which transpired on May 13, 2015 @ around 7:35 pm, at
Brgy. Libas, National Road, San Julian, Eastern Samar, which resulted to damage on my
motor vehicle described as MAKE: ISUZU, BODY TYPE: WAGON, MODEL: 2015,
PLATE NO. AAZ 1284;

To cause and authorize the preparation and filling of any complaints, pleadings, motions,
memoranda, affidavits, and such other papers or documents as may be needed in the
course of the hearing;

To sign and execute all verifications and certifications of non-forum shopping and other
procedural requirements for pleadings filed in my behalf;

To appear for and in my behalf in all mediation proceedings;

To enter into amicable settlements;

To enter into stipulations or documents and to testify in the proceedings/hearings; and

To appoint and secure the services of a legal counsel or attorney-at-law to represent me

in the above mentioned actions or proceedings.

HEREBY GIVING and GRANTING unto my said ATTORNEY IN FACT full power and
authority to do and perform every act and thing whatsoever requisite and necessary to be done in and
about the premises, as fully to all intents and purposes as I might or could do if present, HEREBY
RATIFYING and CONFIRMING all my said ATTORNEY IN FACT may also do or case to be done by
virtue of these presents.
