In the first column of the observation


sheets is where you make a note of the
hazards you have identified as well as
the good practices (do not waste too
much time on good practices as this is
only worth one mark so as long as there
is at least one good practice you can
gain the mark for this). We are looking
for you to identify 30 different hazards.

The next column is the controls section here you must have both short and
long term actions for every hazard you
have noted in order to gain the marks.
The short term actions are those which
you can do straight away to prevent the
hazard from causing harm there and
then.

The timeframes for the controls


measures should be thought about and
should be realistic. If immediate is
written on them all management would
have no idea where to start so this
needs to be thought about carefully.

Each hazard must have the


consequences noted so for example if
you note trailing cables you will not gain
the mark however if you note trailing
cables leading to trip hazards then you
will get the marks for this section.
We are looking for you to identify not
only about 30 hazards but cover a wide
range of hazards so have a look
through and see if you have included
hazards from as many different
categories as possible for example:
- fire.
- chemical.
- transport.
- machinery.
- slips/trips/falls.
- heights.
- health.
- welfare.
- signs.
- first aid etc.

The long term hazards are those which


you are suggesting which should
eliminate of reduce the hazards in the
long term so for example:
If the hazard is a chemical spill caused
when pouring chemicals into smaller
containers from a larger one then the
short term control could be to put up
warning signs to warn people of the spill
and put a spill kit down to absorb the
spill.
However for the long term we are
looking at actions that would prevent
this from happening again so this could
be to contact the suppliers and order the
chemicals in a smaller container so the
need to transfer to a smaller one for use
is eliminated therefore eliminating the
hazard completely.
When thinking about the long term
hazards think of the hierarchy of control
and always start off with those which
would eliminate the hazard completely
and if this is not possible then work
down the hierarchy to the next level.

Some of the short term actions may be


immediate but the long term ones would
need more time - so you can have
different time frames for the short term
controls and long term actions.
You can have timeframes
of one day, a week,
2 weeks, a month etc - students who
simply write immediate for all of their
time frames will not gain the marks as
not everything can be done immediately
- for some controls the company may
have to ask for tenders from other
companies so this needs to be taken
into account.

There is a checklist that you can use to


ensure you cover as many hazards as
possible in the guidance document.

This should start off providing the reader with information about when and where you
completed your inspection.
It should then provide the reader with information about your chosen location - what
the company does and what activities go on there - this should paint a picture to the
reader so they can imagine what hazards they are likely to see in the report.
So note down any separate departments/areas such as offices, workshop, car park
etc. There should also be the activities that go on there so include as much as you
can so this could include:- Use of machinery.
- Use of forklift trucks.
- Handling of chemicals.
- Manual handling of equipment.
- Decanting of chemicals etc.
Please note that you should describe your chosen location to a small degree of
depth so whilst the above list is used as an example, your text should be more
descriptive and should be explained further rather than be in list format.

Executive Summary

This is the first thing management will read from your report so it needs to make
them realise there is an issue. It should therefore summarise the conclusions and
recommendations and make them pay attention and read the rest of your report.
So here there should be a brief overview of the issues and recommendations
needed as well as costs involved and reasons why they should take action - moral,
legal and financial - all in brief as they will be covered in details in other later
sections.

Main findings of the inspection.


Here you should select 6/7 of the main major hazards from your observation sheets
and bring them to your report.
Here you explain the hazards in more details and explaining the hazard, state what
the hazard is and why it is a hazard. You should also state what the likely outcomes
would be should an incident occur.
You would also note the legislation that has been breached - there should be
different legislation noted for each breach and the legislation must be relevant to the
hazard. So for example if there is an issue with manual handling then we would
expect to see the manual handling regulations, if it is in regards chemicals then we
would expect to see the COSHH regulations. We know that HASWA is the umbrella
act and covers everything but we would expect to see the specific piece of legislation
relevant to the individual hazard.
As this is for the International course we would expect to see international legislation
so mainly the ILO regulations.

Conclusions
Here you should summarise the main findings and put a persuasive case forward to
management to make them realise that they have to take action.
So you would be repeating briefly what you found in the main findings. Then you
would need to make management take action so you need to be very persuasive.
Here you should be noting the consequences of not complying.
You should state what can happen so would include improvement or prohibition
notices from the HSE, fines, imprisonment, (stating the value of the fines as well as
the terms of the imprisonment will be beneficial to the report), insurance claims,
increased insurance premiums, loss of morale, lost production etc.
Making them see that the implications of not complying will encourage them to take
action
