1. Introduction
a. >COMP<, hereafter referred to as >COMP< has implemented a Health and
safety program as required by the occupational health and Safety act to ensure
a safe and healthy workplace that will not be of any harm to the environment.
b. The Health and Safety program makes provision that a copy of the following
legislation are made available:
i. Occupational Health & Safety Act 85 of 1993 and its Regulations
ii. SABS 085 - Access Scaffolding (Where applicable)
c. A Health and Safety file with all the necessary legal documentation and
inspection registers for use to comply with the provisions of the regulations will
be available on site for inspection.
d. A personal record of each employee reflecting name, ID, Proof of competence,
medical certificate if applicable is available in the site safety file.
2. Management & Supervision
a. To ensure continuous compliance of the project, the appointed SHE
representative for this site is _____________________.

b. The appointed Site agent(16.2) is,__________________ who will not only be


responsible to manage and co-ordinate the construction work but also the
Health and Safety and environmental protection on this project.
c. The construction supervisor (8.7) is, _____________________ and he will
assist the site agent and have the responsibilities for Health and Safety and
environmental protection as per his appointment in the safety file.
d. >COMP< work procedures ensure that any work on the project is always
carried out under the supervision of the construction supervisor.
e. All appointments of relevant persons are in writing and appointment letters are
kept in the Health & Safety file in the site office.
f. All after hours or weekend work are carried out under competent supervision.
3. Training
a. Site specific employee induction for type of work done by >COMP< are done
before work commences and thereafter for new employees and at least every 6
months.
b. Specific site induction has been developed for employees that work onsite
which includes safety, the requirements and all possible risk(s)/hazard(s} that
exist or may exist on site.
c. Before commencement of new contract the Health and safety advisor,
______________________ will visit the site, do a baseline risk assessment to
ensure that the existing induction training covers all aspects pertaining to the
new site.
d. Thereafter, the continuous risk assessments will be done by the appointed
site/area supervisor.
e. Proof of induction is kept on file at the site.
f. Only competent personnel are employed to work on projects and it is the Site
agent is responsible to ensure that staff are competent for their tasks and are
inducted, instructed and receive relevant training to ensure competence.
g. The company has an ongoing training program driven by the HR department.
4. Public Safety and access control
a. >COMP< Health and Safety program provides for public safety, safe work
areas by pre-planning and setting out correct/appropriate hazard warning signs
and appropriate barricading to ensure safety at all times and the responsible
person is the Site supervisor.
b. Barricading onsite will be of high quality to ensure that hazard and risk areas
are safe to public, vehicles and workers.

c. In accordance with the act the Health and safety program has made provision
to ensure that entry to the site is refused to any un-authorised person or who
appear unfit through alcohol, drug use or any other relevant reason.
5. Agreements with and appointment of sub-contractors
a. The Contracts negotiating team must ensure that a copy of the clients safety
specifications are available to the health and safety officer _______________
at an early stage well before the commencement of the project to ensure that
the safety program for the specific project complies with the specification.
b. It is also the responsibility of the Contracts negotiating team to ensure that the
client provides a signed Section 37 Mandatary agreement to >COMP<. This
document must be signed by the Site agent and a copy placed in the safety file
and a copy returned to the client.
c. >COMP< Health and Safety program makes provision to provide each subcontractor with its Health and Safety specification requirements for the work
specific contract.
d. Agreement and contracts with Mandataries and sub-contractor are compulsory
before commencement of the contract Mandataries or the commencement of
any work by a sub-contractor.
e. >COMP< Health and Safety program requires that each sub-contractor have a
health & safety file onsite in accordance with the Health and Safety
specification presented to them at the tender fase.
f. The Health and Safety file will contain a complete list of all the sub-contractors
on site accountable to >COMP<.
6. Risk Assessments
a. >COMP< Health and Safety program has a comprehensive risk assessments
and safe work procedures program.
b. A competent person, _____________________ as risk assessor have done a
baseline risk assessments before the commencement of work on the project.
c. The identification of all hazards and risks on any project to all workers, subcontractors, visitors, the public and the environment have been completed
before the start of the project by the risk assessor and a daily and continuous
risk assessment will be done by the Site agent, site supervisor, foreman and
staff before any work starts.
d. The project hazards and risks identified are communicated to all staff and proof
of this are kept in the safety file.

e. A documented plan of safe work procedures are drawn up for all normal work
and for any new work with hazards not yet provided for. This plan will indicate
how these risks/hazards are to be mitigated, reduced, controlled and managed.
f. Where applicable, monthly safety audits will take place on behalf of >COMP< to
establish if sub-Contractors Health and Safety plan is implemented and
maintained on the work site.
g. Copies of such reports will be made available in the safety file and to the subcontractor
7. Fall Protection (CR 10)
a. A competent person who has been trained and have experience of the hazards
and risks involved in working at heights, >FP PLANNER< has been appointed
with the responsibility for the preparation of a fall protection plan for the >SITE<
site.
b. The fall protection plans take special care of the following:

All unprotected openings/ledges
Work on scaffolding
Safe use of PPE
Any other points relevant to that specific site

8. Scaffolding (CR 16)


a. Although it is only on rare occasions that >COMP< would work with scaffolding,
all scaffolding operations are carried out under the supervision of the appointed
scaffold supervisor, _________________ and that all erectors and inspectors
are certified and competent to carry out their work.
b. The appointed scaffold supervisor will ensure that scaffolding complies with the
safety standards as per SABS 085 and that it is safe for use prior to
commencement of any work.
9. Electrical Installation (CR 24)
a. >COMP< will ensure that the control of all temporary electrical installations at
the site office is designated to a competent person and that a certificate of
electrical compliance are available in the safety file.
10.

Fire Precautions (CR 29)

a. >COMP< Health and Safety program makes provision that:


b. Appropriate measures are taken to avoid the risk at fire on the site.
c. Sufficient, safe, and suitable storage is provided for flammable liquids and
gasses.
d. Smoking is only allowed in designated fire safe areas.

e. Off cuts, materials, plastics, paper and other materials are not allowed to
accumulate on site but removed on a daily basis,
f. A sufficient number of persons on site are trained in the use of fire extinguishing
equipment
g. There is an effective evacuation plan in place and posted at the site office.
11.

Stacking & Storage (CR 28)

a. >COMP< Health and Safety program makes provision that adequate storage
areas are provided and is under the supervision of the appointed Stacking and
storage supervisor _____________ who shall ensure that;
b. storage areas are kept neat and under control
c. all flammables to be stored separately and the necessary precautions are taken
to prevent a fire or explosion
d. good housekeeping principals are implemented and maintained in storage
areas
12.

Personal Protective clothing and Equipment (Act 8)

a. >COMP< Health and Safety program makes provision for a risk assessment to
be done on site, taking into account the nature of the hazards involved in
working on each specific contract and provision;
b. To limit these risks as far as possible
c. Inform workers and provide them with PPE free of charge
d. Provide continuous training, exercise control and enforce the wearing of PPE
where necessary.
e. Instruct employees in the proper use, maintenance and limitations of the safety
equipment.
f. A document with all of the above information and the signature of the relevant
workers will be kept in the site safety file.
13.

Signage

a. >COMP< Health and Safety program makes provision for the relevant signs are
placed correctly conspicuous areas in accordance with the risk assessment for
example;

14.

Recording, Investigation and Reporting of Incidents (Act 15, 18, 20, 24)

a. Each incident occurring at work or arising out of or in connection with the


activities of persons at work, or in connection with the use of plant or machinery
must be investigated, the root cause established and control measures
implemented, and if the consequence has lead to i. A person;
Dies, becomes unconscious, suffers the loss of a limb or part of a limb or is
otherwise injured or becomes ill to such a degree that he is likely either to
die or to suffer a permanent physical defect or likely to be unable for a period
of at least 14 days either to work or to continue with the activity for which he
was employed or is usually employed;
ii. Or where;
A major incident occurred or the health or safety of any person was
endangered and wherea dangerous substance was spilled, the uncontrolled release of any
substance under pressure took place, machinery or any part thereof
fractured or failed resulting in flying, falling or uncontrolled moving objects or
machinery ran out of control,
b. >COMP< shall report the incident to the provincial Director in the form of
Annex 1 and be completed within seven days of the incident and finalized as
soon as reasonable practicable.
c. Fatalities or very serious incidents will immediately be reported to management,
the Inspectorate and the client.
15.

Health and Safety Representatives and Safety Committees (Act 17 to 20)

a. Health & Safety Representatives have been appointed and trained as per
requirements of Section l7 of the Act.
b. The Health and Safety Representatives for this site is _______________ and
has the duty of doing monthly safety inspection and risk assessments where
necessary of each site and workplace.
c. The results of safety inspections, risk assessments and reports are kept in the
safety file on site.
d. The Health and Safety program makes provision to ensure that where
applicable, all sub-contractors also appoint Health and Safety Representatives.
As per requirement of the act all Health and Safety Reps will form part of the
Safety Committee on each worksite.

e. The Joint Health and Safety Committee will meet bi-monthly and records of
these meetings are kept in the safety file.
16.

Excavation safety (CR 13)

a. The site excavation supervisor, _____________________ will inspect the


excavations and worksite for hazards and potential collapse at the beginning of
each shift and after any incident that could affect the safety of excavations
before any work commences. This inspection will be recorded and kept in the
safety file onsite.
b. The excavation supervisor will be responsible for all safety matter pertaining to
excavation work in his area of responsibility.
17.

Emergency Procedures

a. An emergency contingency plan is drawn up for the site in case of an


emergency.
b. Safe assembly point will be established on-site.
c. Emergency procedures will be communicated to all employees and subcontractors during site induction and toolbox talks.
d. Qualified First Aider _________________and fire fighter, ________________
with relevant equipment are appointed and are available on-site at all times.
e. A list of emergency telephone numbers is placed in conspicuous place in the
site office and in the safety file.
18.

Duty to Inform (Act 13)

a. The Health and Safety program makes provision for >COMP< to inform
workers of any hazards connected to their work, equipment or substance in
their workplace
b. The Health and Safety program makes provision for >COMP< to inform any
Contractor/subcontractor of any changes in the project that might affect the
health and safety of persons that is not covered in the original Health and
Safety Specifications.
19.

Plant, machinery and equipment (CR 23)

a. Only drivers/operators who have a valid operators competence certificates,


drivers licences and certificate of medical fitness will be allowed to operate
plant on the >SITE< sites.
b. The appointed construction plant, machinery, equipment supervisor,
________________ has the duty to ensure that all plant and equipment are in
good, safe working condition at all times before being allowed to work onsite.

c. The plant, machinery, equipment supervisor will also ensure that all equipment
are maintained and serviced in accordance with manufacturers specifications.
d. The plant, machinery and equipment operators are given the responsibility for
daily inspection of their plant, machinery, equipment they operate and report
any faults, unsafe conditions or broken parts to the plant, machinery, equipment
supervisor as soon as possible but at least before the end of the shift.
20.

Demolition work (CR 14)


Demolition work will be performed by >CONTR< and the contractor has
submitted a safety file with a risk assessment. >COMP< site supervisor has the
responsibility to ensure that they perform work in accordance with the section
14 of the Construction regulations 2003.

21.

Waste management

a. Waste management is the responsibility of the site supervisor and all waste
generated onsite will be stored in a safe place and will be removed from site to
a registered dump site on a regular basis.
