Incident no

During pigging operation of caradol sa 34-05 in tk-135 leakage was

observed through 6"ms internal line apx-25-30 ltrs in front of tk-118
dyke crossing.

IN-OLDKAN-11386

Some abnormal sound of short circuit heard while electrical work of

Admin Building First Floor was under progress. Upon notice, immediately
stopped the work and tried to understand the problem. During discussion
it is observed that: Electrical switch board where contractor electrician
were working was live and not isolated LOTO was applied to other MCB
and MCB connecting this switch board was missed for isolation. Vopak
electrician was not able to identify connecting MCB as he was new to this
lighting circuit. PTW and LOTO was applied, however LOTO certificate
was not signed by Vopak Electrical Engineer This is a serious near miss as
the person working could have been electrocuted. This should be further
investigated.

Tanker no-GJ12Z-3942 while loading last compartment and drop wise

leakage observed from manifold flange joint. leakage arrested by

During the tanker loading leakage was observed in delivery line of Tk-102
crossing dyke wall of the tank-110. same was emptied out by air blowing,
pigging and blinded out at tank side. Spilled quantity approx..-25 to 30
litre.

exposed to phenol. His chemical hand gloves tore while performing the
task. Immediately he sprayed the diphotherine solution on his finger as a
first aid measure. Later on propylene glycol was applied to his finger in
fire station.

Vessel MT DAEHO SUNSHINE berthed at OJ-2 @ 0220 hrs on 04.12.14 for

discharging 1041 MT Phenol. Pumping commenced at 0330 hrs. Pumping
for phenol stopped @03:35 hrs due to pump failure of vessel, Then
immediately line blowing done by using vessel air and we were making
arrangements for pigging. But the vessel shown readiness to resume
discharging again & has started pumping @ 03:50 hrs, but stopped again
after running for 15 minutes due to some issue at vessel end, We started
air blowing for hose disconnection but again vessel shown readiness for
discharging & started @ 04:15 hrs with well manifold pressure at vessel
@2.0 kg, We continuously monitoring in the line by hearing the sound
suddenly stopped the sound after cargo passed terminal gate area @0445
hrs, Looking at the abnormality we instructed the vessel to stop pumping.
The pumping of the phenol was stopped @ 0500 hrs & blowing done in
our line. disconnected the line from the vessel manifold. Due to this start
stop activity from vessel end the cargo temperature in dock line dropped
and started solidifying. We introduced water bed through other dock line
and pig the entire dock line back to terminal storage tank.

During general round @ 1615 Hrs, water was found flowing out from the
top of tank 156. First it was thought that the tank is overflowing as water
filling of tank 156 was i/p. But after initial investigation it was found that
the tank level is too low for an overflow and it was found that the water
filling pipe has failed at the welding joint on top of tank 156.

earthing connection using of electrical drill maching. a. AGT is done only
starting of permit then up to 12.40 hours no AGT is taken but Permit

be put in place but no barricading and warning sign found at site.
