take part in the study of Aleta, Maria Rowen, Babao, Stephanie, Descalsota John, Januras
Jeffry, Lazo Patrick, Supnet Cristine Marie,Tibayde Carl leo, Yu Janette entitled The
Role of Internet Trends on the Interdependent View of Self between Male and Female
Adolescents in North Caloocan City.

In following my child to participate in the study, I understand the following;

The Researchers are students completing their thesis as a requirement for Bachelor of
Science in Psychology of University of Caloocan City-Congressional.
The Study is about The Role of Internet Trends on the Interdependent View of Self
between Male and Female Adolescents in North Caloocan City.
The Participation of my child is voluntary and I can withdraw anytime.
The Participation of my child will involve an individual participation on activities and
will answer the given survey questions.
My child has the liberty to express my concerns about my participation and to have my
concerns addressed by the researchers.
The study contains information concerning personal experiences and thoughts but it will
be designed in a manner that anonymity is kept intact.
I understand that my anonymity is protected through the use of pseudonym and the
content gathered from the interview/s will only be used for the research study.
The researchers who will also sign a confidentiality form will transcribe this. No other
person other than the researchers and advisers will have copy and an access to this
recording which will keep in a private storage.
