Assessment of ability, need and readiness to learn:


The physical, psychological and sociocultural characteristics that may
impact his ability to learn as follows: Mr. R is 58 years old. He is alert and
oriented to person, place, and time. He was recently diagnosed with chronic
pancreatitis. He weighs 215lbs and six feet tall making his BMI 29. His previous
diagnosis is Type 2 Diabetes. He has abdominal and shoulder pain as of right now
and on and off migraines. He is experiencing fatigue and diarrhea at this moment.
He teaches at the community college and has a masters degree. His primary
language is English. He is from the Baby Boomer generation. His grown children live
in other states, but has close friends nearby. He learns best by lectures with power
points, printed materials and educational TV channels. He consumes moderate
amounts of caffeine daily.

Identification of problem:

Patient is experiencing chronic pain, priority is to relieve the pain.


His need to know is to learn how caffeine affects the pancreas with the newly
diagnosis of chronic pancreatitis.

Objectives:

The patient will rate his pain scale a 3 or less (on scale of 0-10) after the
intake analgesics as needed for pain in shoulder. He will record the beginning
pain level, the analgesic, time and day, and pain level 60 minutes after

analgesic in a journal every time he experiences pain. He will present the

journal at the one month follow-up appointment to the nurse.


The patient will list to the nurse of beverages not to consume while having
Chronic Pancreatitis prior to discharge.

Interventions:

I will discuss and present printed materials to Mr. R of how to manage chronic
pain. I will include the universal pain assessment tool and a list of his as
needed pain relief medications. I will discuss to him of how and when to use
his analgesics. I will instruct him to write down his pain scale in a journal, also
include the time and day the analgesic was taken, also to include the pain

level 60 minutes after analgesic.


I will present printed materials of beverages not recommended and
recommended to Mr. R. We will discuss the printed materials in great detail of
why it is important to choose beverages wisely.

Evaluation:

During the one month follow-up Mr. R will present a pain journal to the nurse
which listed days and times of when he experienced pain. He included the
day, time, his beginning pain level, analgesic, and his pain level one hour

after taking his analgesic.


Mr. R will present and read back to me of recommended and not recommend
beverages to consume while having diagnosis of Chronic Pancreatitis upon
discharge.
