Obsessivecompulsive disorder (OCD) may be conceptualized as a disorder of selfcontrol and behavioral inhibition.1 Recent
studies2,3 suggestedinterestindeepbrainstimulation(DBS)ofdifferentsubcorticalbraintargetssuchasthenucleusaccumbens,
anteriorinternalcapsule/ventralstriatum,andsubthalamicnucleus(STN)insevere,resistantOCD.STNisacriticalnodeofthe
indirect and hyperdirect basal gangliathalamocortical circuit. STNDBS may release its function as a brake on behavioral
programs,disinhibitinghabitualresponsesandenablingmoreflexiblegoaldirectedbehaviorcontrolinOCD.4,5 Little is known
aboutlongtermoutcomeofSTNDBSinOCD.BasedonexperienceinParkinsondisease,STNDBScaninduceachangefrom
apathetictodisinhibitedbehavior.6 Thistargethasalsoapotentialimpactonaddictions.4,6
