Filling an application for a work permit type C

Documents to be furnished
The application form is to be submitted in Dutch or French only.
1. The application form for a work permit type C of limited duration in 1 original copy, family
name written in capitals.
2. The attached information sheet, adequately filled in and signed by the person concerned,
and signed and dated by the Mayor of the commune where he/she resides. This document is
valid for 3 months.
Next to missing data or modifications a stamp must be applied by the communal
authorities in order to legalise the document.
3. A valid photocopy (recto/verso) of the residence permit of the person concerned (CIRE or
annex 15 issued in anticipation of the legal stay document).
4. An education certificate (model attached) proving that the person concerned is attending a
study.
5. In case of prolongation: a photocopy of the work permit type C.
