Introduction

The purpose of this assignment is to provide a systematic and objective approach to assessing the
hazards and risk associated mining sector in Malaysia and the size of the identified hazards and
provides a method for controlling risk.
The table below shows the corrective and preventive action for accidents that often occur in
mining areas. Assessment is based on the score made from DOSH. Corrective action was based
on improvements that have been taken by the mining and precautions are based on literacy and
guidelines are still not practiced by the mining.

1) Type of incident (Dangerous occurrences at a mine site)

On 21 October 1993 the incident was took near Pantai Remis in Perak, Malaysia. The landslide took
place in an abandoned open cast tin mine close to the coast. This area of Malaysia is well known for
its tin mining industry. That incident was kill one worker and two worker was critical injured. Injuries
caused directly by falling material require great care in classification. Remember that it is the
accident we want to classify. If material was set in motion by machinery, haulage equipment, or hand
tools, or while material is being handled or disturbed, etc., charge the force that set the material in
motion. For example, where a rock was pushed over a highwall by a dozer and the rock hit another
rock which struck and injured a worker - charge the accident to the dozer. Charge the accident to
that which most directly caused the resulting accident. Without the dozer, there would have been no
resulting accident. This includes accidents caused by improper blocking of equipment under repair
or inspection.

deck to see that crusher does not block up

6) Cause (What were the direct, indirect, and root cause)

Direct cause
a rock was pushed over a highwall by a dozer and the rock hit another rock which struck and injured
a worker - charge the accident to the dozer.
Indirect Cause
Supervisor does not alert the worker and work site environment
