AFFIDAVIT OF ARREST
I, MARK P. MORELOS herein undersigned a member of the Philippine National
Police, presently assigned at the Olongapo City Police Office Police Station 3,
Olongapo City, Zambales, do hereby depose and state THAT:
1. As one of the operatives assigned in the entrapment operation, which stemmed
from the same complaint of MARIA HIPOLITO ARAGON for Human trafficking
violations, I personally arrested Alexander Charles Uy for the violation of RA
9208 as amended by RA 10364.
2. Upon arrival at the Hot Babe Club at around 11pm of June 30, 2016 PO2
Cadavis and the other operatives were ushered by Alexander Charles Uy in the
VIP lounge. While they are being entertained and offered women by the latter, I
immediately took a seat nearby the stage afar from the other operatives and
strictly observe the actions of Alexander Charles Uy and other possible victims
inside the said Club.
3. When PO2 Cadavis have taken the offer of Alexander Charles Uy and was
ushered to the showroom, we followed the actions of the latter as he goes back
to entertain another guest/s.
4. After a short-while, I received a text message from PO2 Cadavis that the
operation is a go, while Alexander Charles Uy is talking to another customer and
offering services to, and witnessed, the payment made by the customers. I
personally facilitated the arrest of the latter and informed him of the cause and
nature of his arrest, read him his rights while cuffing him.
5. Then and there Mr. M made no resistance and was apprised of his rights as
provided under the Constitution of the Philippines. He immediately boarded a
vehicle, under my custody, and has been delivered to Olongapo Police Station
for booking and inquest.
I executed this affidavit to attest the truthfulness of the foregoing facts and to
support the filing of Criminal Cases against Alexander Charles Uy for violation of Sec.
4 par. (a) and (e), Qualified by Sec. 6 par. (h) of the R.A. 9208 as amended by R.A. No.
10364
