Ms. M is the sister of the client. She is a fourth year Criminology Student.
She is the fourth child in their family.Sheis close to her brother but days prior to
his admission their relationship went bad.

The second informant is a young nurse working at NDRC. He is the head
nurse in the said institution.
f.) Apparent understanding of present illness of patient:
Mr. L understands very well the condition of the client.He described the client,
taonggrasasiya literally, nungmai-admit dito. Yung as in madumi,
mahabaangkuko at madungistalaga. Hindi siyamakausaptapostulala lang.

a. Maternal and Paternalgrand line age

History
o Dulfos mother side had hypertensive diseases in their lineage.

Dulfos father has diabetes but in their family there were no known familial
diseases.

According to Ms. M, there was history of psychotic episodes on their

fathers side. She verbalized, Sa lahing papa ko, meron. His grandfather
had some episodes of bizarre behaviour but it was undiagnosed.

b. Father

Mr. D is a farmer. He is very hardworking and good provider. According to

Ms. M, his father is friendly with people outside their house, but is very
strict and aloof at home. Though he acts friendly, but he doesnt trust
them. He is also a very protective father.Ms. M verbalized, Tong bata pa

He was also the type who hurt his children whenever they commited

Mrs. D is a high school graduate. She didnt have any job before, but

when she got married, they started a business which is a store and a farm.
According to Ms. M, she is a loving mother. For them, she is their Super
Woman. She is always protecting them whenever their father is scolding
them.

d. Siblings

Their first child is Dulfo, our patient, is already 24 years old. He was
helping in their family business as a driver and sometimes he

The second child is 23 year-old female and works as a nurse.
Then, third child, 22 years old, female is also BS Nursing graduate

and currently reviewing for licensure examination.

Then the fourth child (informant) is a 19 year old female and

currently studying BS Criminology at SMIT, Tacurong City.

Then the fifth Child, female, 18 years old is Studying BS education

also at SMIT, Tacurong City.

Youngest child is a male and is three years old.

Personality History
a) Prenatal
When DULFO was conceived, his parents were very happy and excited.
Her mother really took care of him throughout the pregnancy, since he is
the first child.
b) Birth
DULFO was born on January 24, 1989 via normal spontaneous
vaginal delivery without complications. He received complete
immunization.
c) Infancy and Childhood Characteristics
Dulfo was breastfed and then bottle fed when his mother became
busy with their business. Around two years old he was then being
care by a nanny.

According to Ms. M, DULFOis not playful because their father

doesnt want them to mingle with others.Dulfo started to feel shy to

everyone. During hischildhood, he didnt have any friends. He spent

a lot of his time reading books. Her sister, Ms. M, verbalized, Tong

Mr. was afraid of their father when he was a child. There was a time
when he lost the watch their father gave to him, he received a
spank and lectures from him.

When he was in elementary he had an accident, Ms. M said,

very in love with her.

He didnt have friends to play with because his father doesnt want
him to, so he almost isolated himself from others. Though he is not

f. School History
Dulfo started entering school at the age of 4. He was enrolled at Kulaman
Central School. He had gain friends by this time, but he didnt develop trust with
his friends because their father brought them up that way. He became active in
any school activities and eventually he became top of their class. He loves
reading, there were times that he stole books in their library and brought it at
home to read.
Back in high school, he enrolled at Notre Dame of Kulaman. He became
top of the class during his first and second year of high school. On third year, he
became addicted with computer from the influence of his friends. There was a
time that the son of Principal did bully him. He wants him to make his
assignments then he refused and he got punched by that person, then he started
not to attend classes and stay at the computer shop. He avoided all his friends
and eventually dropped from school. He did not attend school for 1 year. He
transferred to another school, then he suffered another maltreat from the
husband of her teacher, according to Ms. M, sir Dulfos teacher had a crush on

him that why the husband of the said teacher punched him. He was afraid of his
parents that he kept it a secret from them but eventually they found out because
of the bruises. He did not enter college because he wants his siblings finish first.
g. Religious and Social Adaptability
When he was a child, Mr.Dulfo is friendly but during his high school years
He became suspicious of other people. He doesnt trust them. He suffered
from bullying when he was in high school.
As for religion, he believes in God but he doesnt go to church because
the sermon were the same as always, as Ms. M verbalized

Towards failure and success Mr.Dulfo is very frustrated. He has a lot of
dreams when he was a child but none of them came true.
h. Occupational history
Mr.dulfo works as a driver for their store. Sometimes he also watch their store.
He very workhaholic. There were times that he will just work and will not eat food.

i. History of substances use:
Mr.Dulfo has no known history of substance abuse.
j. Onset of present Illness

According to Ms.M,Everything started when there was a death threat

for their family. It was around March of 2013, in Sinuksok, where
Dulfos familys farm is located, there are illegal loggers and their saw
were confiscated by the government. The former turned their anger to
Dulfos family. They threatened them, if they will not pay for the saw
they kill their family. If they will pay 80,000 they let their family live.

Because of what happened Mr.Dulfo got mad. They went back to

Kulaman. Then, after few days, he became more suspicious of people
around him. he began to get frightened of the people. He feels like all
people around him will kill their family and he kept on giving warnings
to his family. It was good Friday that time.Whenever there are visitors
in their house, he wont let them to come inside because he doesnt
trust of them. One time, his father went to Sinuksok then came back to
Kulaman, Dulfo got mad, and he doesnt want to leave their house
without a person to watch for it. So he stayed there(Sinuksok) for a

week. He did not eat throughout his stay there.

After his stay at Sinuksok, He was afraid that the sun keeps on
following him. he told her that on third day, all of people will die. He will
be the person to stay alive because he is immortal. He doesnt want to
ride on cars because he sees it and the people as aliens. Then he is
always staring at the sun, moon and stars. Mr.dulfo doesnt eat
because he believes he is immortal. He also said that he is a scientist
and is very bright. The genius type since he can see that face of the
sun. he they let him say what he wants because they were afraid he
will stow away. He accused his younger brother as gay. Whenever he

sleeps, he let the doors and the windows open.

That night, he ran away and Miss M followed him, they talked and had
a joyride afterwards ,after he drove her home he goes somewhere else
. The following day he went back home . His sister tried to talk to him
because they taught that he doesnt want to go home . But then they

were wrong because he only went to the grassy area near their house.
He didnt eat anything that night before he went away from their home .
Her sister asked him what did he eat hilamon? Hilamon gin kaon mo?
He had no response. But suddenly her sister noticed that there were
pieces of grass left in his teeth. In the afternoon during that day his
father hired a service van and his father asked him if he wants to go
swimming at General Santos City. When the service van came, His
father convinced him to go with them in General Santos City, but he
dont want. He punched his father back instead and he ran away, trying
to escape, thinking that it was a bait for him. His father called for help
to catch him. He asked them that if they will catch him they will tie him
up. Those people agreed to help him. The former invited Mr. Dulfo to
play basketball as a bait. After they played, they asked him to go to
General Santos City. He agreed and went with them, since he already
realize that he has no choice but to come. When they arrived in
Gensan , they went to the beach to go swimming, So there whenever
he hear his favourite song he will laughed and he suddenly cry. After
that he saw a lizard he caught then ate it . Afternoon of the next day
They went to the hospital while waiting they went to the cafeteria to
have snacks, then he saw a spider and he ate it . at the cafeteria he
saw a spider . Upon entering the hospital he saw a sign board saying
Psychiatric Clinic since he know what it meant he got mad at them .
He thinks that there is nothing wrong with him. He ran up to the 3 rd floor

of the hospital. He wanted to jump. He wasnt able to ,because the

policeman caught him. Then they give a shot of injection . When hes
already calm . He had an interview .

They sought the help of a psychiatrist in General Santos City on the

same month, and then he attempted to get away from them. He lost his
trust for his parents. He got at them. Then they managed to get him. he
was given an unrecalled injectable drug. And the Doctor, referred them

to bring him to NDRC for further treatments.

The next day they immediately go to Davao City but they do not know
the way to the New Day Recovery Center. His father and her sister
bought a load to contact the exact way to NDRC. Mr. Dulfo and one
accompanying relative were left inside the van. Mr. Dulfo pretended
that he needs to pee, but instead he picked up a wood and hit his
relative. He escapes and he was caught at Toril Davao City. He
pretended that he is insane for him to set free in jail . He was walking
on his way back home.
