Introduction:
This document contains the general policy and guidelines for the Bachelor of Science in
Civil Engineering On-The-Job Training program and will take effect this 1st semester,
school year 2016-2017.

I.

BSCE OJT POLICY AND GUIDELINES

GENERAL OBJECTIVE OF THE PROGRAM

In response to the demands of the industry for globally competitive Engineering


graduates as to knowledge, skills, attitudes and values, the OJT aims to:

a. Give the student exposure to the actual technical and managerial aspects of
the CE profession.
b. Enable the student to gain experience, relate and apply the theories he learns
in school to real industrial situation and problems.
c. Develop and instill a positive attitude, self-confidence and self-motivation
requires of a responsible professional in handling tasks.
d. Impress on the student the importance of human relations in the working place
or environment.

II. GENERAL REQUIREMENTS OF OJT

BSCE OJT POLICY AND GUIDELINES
a. All CE students enrolled in CMPM class must undergo OJT.
b. A student must be fifth year standing and finished at least 80% of major subject
from fourth year,
c. Registrar evaluates if he/she is qualified to enroll the subject.
d. Students will provide necessary documents before OJT starts.
e. OJT students and guardian must be fully orient prior to training
f. The minimum required number of hours of training is 100.
g. A student is required to submit his/her attendance, accomplishment form and a
weekly report.
h. The student will be evaluated by his/her supervisor from the company
periodically.
i. A student is required to submit a final report at the end of his training.
j. Must submit research paper regarding any topic or improvement or proposal to
the company.
k. Every company have authorized signatory to their evaluation form.

II. PROCEDURES AND GUIDELINES


a. The student writes a LETTER OF APPLICATION addressed to the company
concerned. The student may send to more than one company.
b. The student secures his LETTER OF RECOMMENDATION or LETTER OF
ENDORSEMENT from the College Dean or in the absence of the College Dean,
from the OJT Coordinator/Adviser.
c. The student hand-delivers or mails the letter to the company and waits for
acceptance. The student is responsible for following-up his LETTER OF
APPLICATION with the company.

d. When the company accepts the student for OJT, the company must fill up the
LETTER OF ACCEPTANCE which the student must collect and submit to the OJT
Coordinator/Adviser before the start of his/her OJT.
e. The OJT Coordinator/Adviser must in turn coordinate and confirm with said
company regarding the students acceptance to undergo OJT with them. The
College Dean and the companys representative should agree on the period and
schedule of the students OJT through signing of MEMORANDUM OF AGREEMENT.
f. The student then submits the WAIVER FORM signed by his/her parent or guardian
to his/her adviser.
g. The student must submit a copy of the following documents before the starts of
his/her OJT: LETTER OF APPLICATION< LETTER OF RECOMMENDATION OR LETTER
OF ENDORSEMENT< ACCEPTANCE LETTER> CLEARNCE FROM SAO>GOOD
MORAL>PERSONALITY
TEST>MEDICAL
CERTIFICATE<MEMORANDUM
OF
AGREEMENT and WAIVER FORM.
h. The student can start his/her OJT on the recommended date.
i. If required, the student submits a Training Program from the company upon
acceptance.
j. The student is required to make a progress report, on a day-to-day basis and
submit it every Saturday to the OJT adviser.
k. Upon completion the student is required to obtain from the company several
documents (chapter X)
l. The student is then required to submit a hardbound written report a outlined in
chapter IX.
m. Students must seek approval from the Department Chair if work (company) is
outside of Oriental Mindoro. Violation of this will merit deductions from the grade
of CMPM.
n. The student must record his daily attendance indicating the arrival time and
departure time. The supervisor must also have it signed daily. The DAILY
ATTENDACE RECORD must be submitted to the OJT Coordinator/Adviser on a
weekly basis and must be compiled and attached in the FINAL REPORT.
o. The student is required to make a WEEKLY REPORT that encompasses the daily
accomplishments or daily activities. The OJT Coordinator/Adviser checks this
every Saturday during the designated class schedule.
p. The student should at all times keep all documents pertaining to his/her OJT.
However, the OJT Coordinator/Adviser must be informed of any new documents or

forms that were accomplished in relation to his/her training. He/She compiles all
these documents for his/her final report.
q. The student must comply with the general guidelines of the company regarding
their own OJT program policy.
r. The student must complete at least the minimum 60 hours in one company. If for
any valid reason, he/she cannot comply with the 60 hours in one company,
he/she has to look for another company and complete the remaining hours there.
This has to be done upon approval of the OJT Coordinator/Adviser and the College
Dean. Chapter III Sections C until Section P must also be repeated by the student.
s. While the student is on OJT, the OJT Coordinator/Adviser may go for surprise
visit and evaluation.
t.Upon completion of the minimum 60 hours and the tasks given by the company,
the student is required to obtain from the company the following documents: (a)
CERTIFICATE OF COMPLETION and (b) FINAL EVALUATION FORM.

IV. DUTIES AND RESPONSIBILITIES OF A HOST COMPANY

a. Through the immediate supervisor or through the Human Resource Department


or equivalent office, ensures that the OJT a student undergoes is technical in
nature and related to CE.
b. Ensures a safe working environment for student undergoing OJT.
c. Assigns an immediate supervisor who will monitor the attendance and
performance of the student for the duration of the OJT, in coordination with the
OJT Adviser.

d. Assigned supervisor will have the responsibility of monitoring the activities and
tasks performed by the student including the signing of his/her daily attendance
and accomplishment and perform periodical assessment on the progress of the
student.
e. Upon completion of the required number of weeks, requires the student to
submit to the OJT Coordinator/Adviser a Certificate of Completion stating the
students general performance and that the student has satisfactorily completed
all requirements.
f. Through the assigned supervisor, fills-up the Evaluation Form, and signs and
seals and submits this to the OJT Coordinator/Adviser through the student.
g. Informs the school through the OJT Coordinator/Adviser and/or the College
Dean of the matters related to the students OJT program in the company.

V. DUTIES AND RESPONSIBILITIES OF THE COORDINATOR/ADVISER


The OJT Coordinator/Adviser is responsible for taking the necessary actions in
briefing the students and providing them the guidelines in the performance of
their duties.
Following are the responsibilities of the OJT Coordinator/Adviser:
a. Through the immediate supervisor or through the Human Resource Department
or equivalent office, ensures that the OJT of a student undergoes is technical in
nature and related to CE profession.
b. The OJT Coordinator/Adviser updates forms relating to OJT.

c. The OJT Coordinator/Adviser establishes communication with the student


trainees Supervisor during the OJT hours
d. The OJT Coordinator/Adviser performs managing and evaluation of the student
trainee.
e. The OJT Coordinator/Adviser computes the final grade of the student based on
the existing Grading System.

VI. DUTIES AND RESPONSIBILITIES OF THE PARENTS/GUARDIAN


The Parents/Guardian of the student is/are the partners of the OJT
Coordinator/Adviser and the Host Companys Supervisor in monitoring the
activities and accomplishments of the student.
Following are the responsibilities of the Parents/Guardian:
a. To encourage the student to carry out effectively his/her duties and
responsibilities.
b. To share responsibilities for the conduct of the student while in the program.
c. To be responsible for the safety and conduct of the student-learner while
travelling to and from school the OJT site and home.

VII. DUTIES AND RESPONSIBILITIES OF THE STUDENT


The student/trainee must always observe discipline and right conduct. The student
must also abide by the following duties and responsibilities:
a. The student/trainee should exhibit good image.
b. The student/trainee should not engage in any form of intimate relationship with
clients, staff and on-site intern supervisor.
c. The student/trainee is not allowed to invite/entertain personal visitors in the OJT
site.
d. The student/trainee should avoid incessant talking and loitering in the OJT
premises.
e. Cellular phones and other electronic gadgets should be turned off or set on a silent
mode when he/she is on duty.

f. The student/trainee should not engage in any form of gambling/drinking
intoxicating beverages and the like while on duty or off duty within the OJT premises.
g. The student/trainee must comply to OJT guidelines and procedures provided by the
OJT Coordinator/Adviser.

VIII. GRADING SYSTEM


The student will be graded based on his performance and compliance to the policies
and guidelines provided. The following statements will be the grading system for the
duration of the students OJT.

(d)MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and entered into by and between:

The _(Company_), a private firm duly organized and existing under


the laws of the Philippines, with principal office address at
Kilometer 5, Sta Isabel, Calapan City herein represented by
(Contact Person/Manager/HR) to represent in this transaction,
hereinafter referred to as the_(Company_).

The DIVINE WORD COLLEGE OF CALAPAN,


an educational institution
duly organized and existing under the laws of the Philippines with
principal office at Gov. Infantado St., Calapan City, herein
represented by Vice President of Academic Affairs
who is duly
authorized to represent in its transaction, hereinafter referred to
as DWCC.
WHEREAS, the_(Company_)shall form a partnership with the DWCC as part
of its effort to provide training ground/opportunities and to cultivate a
pool which, it might recruit needed staff and which are required by the
subject as prerequisite for its completion.
WHEREAS, the DWCC is offering Bachelor of Science inCivil Engineering
(BSCE), wherein its students are required to undergo on-the-job training as
a requirement of the subject CMPM.

NOW, THEREFORE, for and in consideration of the foregoing premises,
_(Company_)and DWCC do hereby agree and stipulate the following:
A. (Company) shall:
1.

Screen, select and deploy to the different offices and


operating units of the _(Company_)students recommended by the
DWCC to undergo on-the job training pursuant to the following
conditions:
1.1 Minimum of 150 hours and maximum if 250 hours for on- thejob training in their ideas of specialization;
1.2 Inclusion in any educational programs for on-the-job
trainees sponsored or initiated by _(Company_)such as
Career Orientation and etc.;

2.

Provision to the DWCC any materials, products, technology of


Globe Telecom that may be useful to be presented during the
annual DWCC Foundation Days participated by the whole college and
can be seen by the industry and most commercial establishment in
the province of Oriental Mindoro.

3.

Assign personnel to take charge and supervise the on-the-job


training and community extension activities of students, it being
understood that they will be assigned/required to perform duties
and functions along their areas of specialization;

4.

Issue certificates of completion to students who successfully


completed their training under this program; and

5.

Adopt and enforce rules that will govern the conduct of the
program.
Said rules shall be made known to the DWCC and the
student trainees before the start of the training.

B. DWCC shall:
1. Pre-select and recommend students who will undergo on-the-job
training, it being understood that only students taking BSCE
course will be recommended, wherein this training is an academic
requirement for the subject CMPM.
2. Submit to _(Company_)whatever documents are required under this
program;

3. Together with the students and parents concerned, renounce, and
waive any claim against the _(Company_)for any injury that the
students may sustain or loss that they may suffer, personal or
pecuniary, in the performance of their duties and functions while
under training.
The waiver of the student trainees shall be
submitted to the DWCC prior to the commencement of the training;
4. Hold the _(Company_)free from any claims or suit and shall
forever depend the _(Company_)from any such claim or suit
whatsoever in connection with this Agreement.
C. (Company) and DWCC further agree that:
1. It is expressly understood that there will be no employer-employee
relationship between the student trainees of the DWCC;
2. The student trainees will be personally responsible for any and all
liabilities for damage to property or injury to third persons which
may be occasioned by their intentional or negligent acts while in
the course of their training;
3. The student trainees and the DWCC Program Coordinator shall abide
with the _(Company_)rules and regulations and comply with those
imposed for the program; otherwise they shall be excluded from
further participation;
4. The _(Company_)reserves the right to discontinue the on-the-job
training and community extension activities of any student-trainee
upon prior written notice to DWCC;
5. The DWCC may pull out any student from the program upon written
notice to the _(Company_).
D. DURATION
This agreement shall take effect immediately upon signing hereof
and shall continue to be in force and effect until such time that
any one of the parties withdraws from this Agreement upon seven
days prior notice to the other.
