Behavior

The patient follows and obeys and follows my requests but he gets easily distracted
by sounds and stops talking during conversations and only talks again when called
or tapped at the knees.

Emotional tone

The patient is always in a calm manner and sometimes with a happy smile during
conversations that interests him.

Thought Process

The client participates in a conversation which he likes or any topic that would
interest him in talking otherwise the client wont participate in the conversation. The
patient also gets easily distracted by any sound like clanging of the metal part of
the stethoscope and even when there are others talking near him, he usually looks
at the source of the noise and stares at it until tapped on the knees for getting back
his attention.

Content

The client said that he experiences delusions and hallucinations when he forgets to
take his medicine. The client stated that he sees the shadow of a man with a devils
horn and he also hears someone shouting at him and saying bad words to him.
