CERTIFICATE OF ONENESS

I, TARHATA M. MUSA, of legal age, Filipino, married, and a resident of

Purok Hiso, Kalawag III, Isulan, Sultan Kudarat, Philippines, hereby depose
and state:
1. THAT my maiden name is Tarhata A. Mopac as stated and registered
in my birth certificate issued by Office of the Civil Registry. A copy of my
birth certificate is hereto attached as ANNEX A;
2. THAT in my previous filing in PAG-IBIG loan, my surname was
erroneously written as "MUPAK" instead of my true and correct surname
of "MOPAC";
3. THAT my true surname is "MOPAC"; as appearing in the same
Certificate of Live as the surname of my father;
4. THAT in all pertinent documents which I had used, to include that of
my scholastic records (transcript of records), I used my true and correct
surname of "MOPAC";
5. THAT I am executing this Certificate Of Oneness to attest to the truth
of the foregoing facts as regards my true surname.
