The product described herein has been manufactured in compliance with IVD
directive 98/79/EG.
Na-Azide may react with lead and copper plumbing to form explosive metal
azides. On disposal, flush with a large amount of water to prevent azide build-up.
The calibrators and controls contain components of human origin. They have
produced negative results when tested for Human Immunodeficiency Virus
(HIV)-Ag, hepatitis B surface (HBs)-Ag, HIV 1/2-Ab and hepatitis C Virus
(HCV)-Ab, in FDA-approved or European Directive 98/79/EG-compliant tests.
However, no known test can guarantee that products derived from human blood
will not be infectious. They should therefore be handled as if capable of
transmitting infectious agents, and discarded appropriately. Please refer to
CDC (Center of Disease Control, Atlanta, USA) or other local/national
guidelines on laboratory safety and decontamination procedures.
b. Sample buffer, 100 mL, ready-to-use, orange coloured. Contains Trisbuffered saline (TBS), bovine serum albumin (BSA), Tween and Na-azide.
c. Wash buffer, 100 mL, 10x-concentrate, blue coloured. Contains TBS, Tween
and bromonitrodioxane.
e. Negative and positive control, 2,0 mL each, ready-to-use, green and red
coloured, respectively. Contain TBS, BSA, Tween and Na-azide.
techniques and dilute them 1/100, e.g. 10 L serum + 990 L sample buffer.
Mix thoroughly.
For rapid dispensing during the assay procedure, preparation of the
calibrators, controls and samples in microwell transfer tubes is
recommended. This allows the operation of an 8-channel pipette during the
assay procedure.
If samples are not assayed immediately, they should be stored at 2 - 8C
and assayed within 3 days. For longer storage, -20C or lower temperature
are recommended. Repeated freezing and thawing of sera should be
avoided. Thawed samples must be mixed prior to diluting.
Specimen requirements: Highly lipemic, haemolysed or microbially
contaminated sera may cause erroneous results and should be avoided.
8. Assay procedure
8.1. Manual operation
Before starting the assay, all components of the kit must have reached room
temperature (23 3C).
To achieve best results, i.e. the maximum ratio between specific and
background signal, careful washing is essential (steps a, c and e). It is
crucially important to remove the wash solution completely. For that
purpose, tap the plate firmly on several layers of absorbent tissue. Automated
washers must be verified according to results obtained by manual washing.
a. Immediately prior to use, wash the solid phase once: fill wells with 350 L
wash buffer each, soak for about 10 seconds in the wells and remove.
b. Dispense the calibrators (2,0 mL each, ready-to-use, gradually blue),
controls (2,0 mL each, ready-to-use, green and red) and the diluted samples
rapidly into the microwells; 100 L per well. Duplicate measurements are
recommended.
Incubate the plate for 30 minutes at room temperature (23 3C).
c. Wash the wells 4 times as in step a.
d. Rapidly (preferably using an 8-channel pipette) dispense the conjugate (14
mL, ready-to-use, red); 100 L per well. Incubate the plate as in step b.

Quality control: The positive and negative control check the assay performance.
Their authorised values and acceptable ranges, respectively, are quoted in the
lot-specific certificate of analysis. Values of the controls have to fall within the
indicated ranges; otherwise, the results of the assay are invalidated.

In order to assess the dose-response relationship of the test, positive sera were
measured in serial 2-fold dilution. Acceptance criterion: linear regression of 4
successive dilutions must yield a correlation factor > 0,98. A typical result is
depicted below.

ROC-analysis of these data was used to determine the cut-off as 40 IU dsDNAAb (IgG)/mL (7). The data presented here suggest a diagnostic specificity and
sensitivity of the ELISA of about 98 % and nearly 100 %, respectively. These
values apply for the measured sera only; other collectives may yield different
results.

Na-Azid kann mit Kupfer- und Bleirohren reagieren und explosive Metallazide
bilden. Beim Entsorgen mit Wasser nachsplen, um eine Akkumulation zu
verhindern.
Die Standards und Kontrollen enthalten Komponenten menschlichen
Ursprungs. Sie wurden daraufhin geprft, ob Human Immunodeficiency Virus
(HIV)-Ag, Hepatitis B-Oberflchen (HBs)-Ag und Antikrper gegen HIV 1/2 und
Hepatitis C-Virus (HCV) vorliegen und zeigten negative Resultate; entweder in
einem FDA-zugelassenen oder einem CE-konformen Test, entsprechend der
Europischen Richtlinie 98/79/EC.
Allerdings kann kein Test garantieren, dass Material humanen Ursprungs
tatschlich nicht infektis ist. Die Prparate sollten daher als potenziell infektis
behandelt und entsprechend entsorgt werden, gem CDC (Center of Diseae
Control, Atlanta, USA)- oder anderen lokalen / nationalen Richtlinien zu
Laborsicherheit und Dekontaminierung.
3. Testprinzip
Die Kavitten der Festphase sind beschichtet mit dsDNA. An dieser Oberflche
laufen die folgenden immunologischen Reaktionen ab:
1. Reaktion: dsDNA-spezifische Antikrper aus der Probe binden an das
immobilisierte Antigen; es bildet sich der Antigen-Antikrper-Komplex. Nichtgebundene Probenbestandteile werden anschlieend von der Festphase
gewaschen.
2. Reaktion: Ein zweiter, gegen human-IgG gerichteter und mit Peroxidase
(HRP) konjugierter Antikrper wird zugesetzt. Dieses Konjugat bindet
seinerseits an den Antigen-Antikrper-Komplex. berschssiges Konjugat
wird anschlieend von der Festphase gewaschen.
3. Reaktion: Der Enzym-markierte Komplex setzt ein farbloses Substrat in ein
farbiges Produkt um. Das Ausma der Farbentwicklung spiegelt die Menge
an dsDNA-Antikrpern (IgG) in der Probe wider.

Diese Spezifikationen sind nur als Anhaltspunkt zu verstehen. Zu ihrer
berprfung sollten in jedem Test Normalseren mitgefhrt werden.
Ein negatives Ergebnis zeigt an, dass der Patient keinen erhhten Titer an IgGAntikrpern gegen dsDNA aufweist. Sind jedoch charakteristische klinische
Anzeichen des SLE erkennbar, sollten weitere anti-nuklere Antikrper
bestimmt werden.

11. Testcharakteristika
11.1. Standardisierung
Der Test wird mit einem gereinigten Serumprparat standardisiert, das
spezifisch gegen dsDNA gerichtete IgG-Antikrper enthlt. Es wird seinerseits
kalibriert am 1. internationalen Standard fr dsDNA-Antikrper mit der
Bezeichnung Wo/80. Der Reaktivittsgrad einer Probe wird in internationalen
Einheiten (IU dsDNA-AK (IgG)/mL Serum) angegeben.
11.2. Analytische Spezifitt
Der Test weist spezifisch humane IgG-Antikrper nach, die gegen dsDNA
gerichtet sind.
11.3. Nachweisgrenze (analytische Sensitivitt)
Die Nachweisgrenze ist definiert als diejenige Konzentration des Analyten, die
dem OD-Mittelwert des Probenpuffers entspricht, zu dem die 3-fache
Standardabweichung (s) addiert wurde. Sie wurde zu < 1 IU dsDNA-AK
(IgG)/mL Serum bestimmt (n = 24).
Empfohlener Messbereich: 5 - 250 IU dsDNA-AK (IgG)/mL Serum.
11.4. Festphasen-Homogenitt
Dieser Parameter ist regulrer Bestandteil der QC jeder Produktions-Charge.
Die Homogenitt wird bestimmt durch 288-fache Messung einer positiven, aber
nicht sttigenden Probe auf 3 ausgewhlten Platten. Akzeptanz-Kriterium:
mOD-Variationskoeffizient (VK) ber die Platten < 8%. Die folgende Abbildung
zeigt einen reprsentativen Auszug einer solchen Analyse (Ch.-Bez. der
Festphase: 0809C).

15. Kurzanleitung
a. Die Seren 1/100 in Probenpuffer (100 mL, gebrauchsfertig, orange)
verdnnen und durchmischen.
b. Das 10x-Konzentrat des Waschpuffers (100 mL, blau) mit Wasser
verdnnen und durchmischen.
c. Die Kavitten der Festphase einmal mit je 350 L Waschpuffer waschen.
Dann je 100 L der Standards (je 2,0 mL, gebrauchsfertig, abgestuft blau),
der Kontrollen (je 2,0 mL, gebrauchsfertig, grn bzw. rot) und der verdnnten
Proben in die Kavitten pipettieren. Doppelbestimmungen sind zu
empfehlen. 30 Minuten bei Raumtemperatur (23 3C) inkubieren.
d. Die Kavitten 4x mit je 350 L Waschpuffer waschen.
e. Je 100 L des Konjugats (14 mL, gebrauchsfertig, rot) in die Kavitten
pipettieren. Inkubieren wie in Schritt c.
f. Waschschritt d wiederholen.
g. Je 100 L des Substrats (14 mL, gebrauchsfertig, in einem schwarzen
Flschchen) in die Kavitten pipettieren. Inkubieren wie in Schritt c. Dann je
100 L Stoplsung (14 mL, gebrauchsfertig, farblos) zusetzen und die Platte
kurz schtteln.
h. Sofort die Absorption bei 450 nm messen.
i. Quantitative Auswertung: Die Standardkurve ermitteln und anhand dieser
Kurve die Absorption der Proben in ihre jeweilige Antikrper-Konzentration
(IU dsDNA-AK (IgG)/mL Serum) umformen.
j. Qualitative Auswertung: Die grenzwertige Absorption ermitteln, indem die
Absorption der positiven Kontrolle mit dem Faktor multipliziert wird, der im
Analysen-Zertifikat angegeben ist. Dann die Ratio-Werte der Proben
berechnen, indem ihre Absorption durch die grenzwertige Absorption
dividiert wird.
