Site visit was conducted on SLOK storage facility at ikot-Abassi in Akaw ibom
state in regards to the invoice No. STG/09-15/SLK/002 dated on the 26 th of
October 2015.
ITEM 1
Product tanks and pipelines installation of pipeline flexibility
DESCRIPTION
To allow for flexible use of the pipeline in discharging multiple product in the
loading gantry
OBSERVATION
AGO and DPK products have dedicated pumps and dedicated loading arm
respectively, the purpose of the said pipeline flexibility is to allow for
maximum loading of the two product using the product pump and the
loading arm to load either of the two product. Interconnecting the AGO
product line with DPK product line at the pump shed, this will allow for
flexibility in loading AGO product with both AGO and DPK product pump with
their respective loading arm likewise for loading DPK product with both DPK
and AGO product pump with their respective loading arm when only one of
the two product is available.
RECOMMENDATION
Engineering drawing is required for the modification scope of work and
adequate bill of materials should be generated to allow for adequate costing
of the materials needed for the modification scope of work. The invoice
generated cannot be quantified with the said scope of work, without
engineering

drawings,

authentication

ITEM 1.3
Radiographical x-ray conduction of pipeline
DESCRIPTION
To check for leakages along the joints and the entire length of the pipeline
system

OBSERVATION
There is no physical evidence but the certificate presented on site to show as
evidences of work done for calibration of the tanks nos and x-ray report for
the pipeline weld joints. The number of weld joint can only be verified with
the availability of piping general arrangement drawings and the fabrication

isometric drawings which were not made available as at the time of the visit.
Engineering drawings for the storage facility cannot be produce as at the
time of h visit, but tank calibration and x-ray report can be requested from
the company.
ITEM 3.1
Reconstruction of the collapsed culvert on the temporary access road
OBSERVATION
The present culvert on the access road was badly constructed due to nonavailability of structural drawings for the culvert, likewise the mix ratio used
cannot be determined and the rein-enforcement member used for the
construction of the culvert are not adequate.
RECOMMENDATION
Structural drawings for the damage culvert must be engineered before
embarking on re-construction. Engineering drawings should be generated,
showing

the

required

rein-enforcement

member

and

mix

ratio

to

accommodate the required external load from heavy vehicular movement.


Engineering drawings will also be used for generating the bill of materials
which will be used for project costing. The invoice generated cannot be
quantified with the said scope of work, without engineering drawings,
authentication of the cost required for the modification scope of work cannot
be determine.

ITEM 3.2

Reconstruction of collapsed perimeter fence


OBSERVATION
Reconstruction of collapsed perimeter fence is necessary, but be that as it
may, topography of the storage facility needs to be adequately managed to
allow for proper flow of storm water within the storage facility. The collapsed
fence was due to the erosion activities within the facility. Without addressing
the erosion activities in within the facility, no amount of construction works
will make the perimeter fence to stand the test of time.
RECOMMENDATION
The tanks foundations are under serious treat if the continuous degrading of
the storage facility is not arrested. Bill of engineering measurement must be
carried, and structural drawing for the fence foundation in order for it to
withstand pressure from storm water. The invoice generated cannot be
quantified with the said scope of work, without engineering drawings,
authentication of the cost required for the modification scope of work cannot
be determine.
Below are attached pictures of the collapsed fence and topography
degradation of the storage facility.

ITEM 4
Product inventory management optimization (level gauging system, quality
analysis and control)
OBSERVATION
Work is ongoing on site, as said by the site engineer, no physical evidence on
site.
GENERAL OBSERVATION
1. Installation of CCTV camera and perimeter lighting were ongoing as at
the time of visit to the site.

2. Installation of 10Kva inverter with 15 batteries.

3. Firefighting equipment for the product storage facility has been
compromised with improper firefighting equipment that was locally

fabricated rather than purchasing standard firefighting equipment.


Foam tank is a devise that supplies the firewater system with foam;
this tank is size based on the volume of product stored and comes with
associated accessories for easy pipe connection.
4. Location of the fabricated foam tank is too close to the storage facility
bond-wall, neglecting the safety standard practice in product storage
facility. Ability to operate the foam tank in case of any eventuality is
not realistic.

5. Firewater pump left un-installed, one out of the two firewater pump
that was provided was installed, and the act of not installing the other
firewater pump is making the storage facility not safe for receiving
product and dispensing of petroleum product.

6. Pipe penetrating the bond-wall without sealing the open ends of the
sleeves in which the pipe went through will always give access to spill
product rather than containing the product in within the bond-wall in
case of storage tank rupture.
