AFFIDAVIT OF NON-EMPLOYMENT
I, LYJOE LAUS TOLEDO, 17 years old, Filipino, single, with address
at Kadaklan Village, Apugan, Baguio City, on oath, states:
1. I am currently studying as a fourth year high school student at
the Mil-an National High School, Loakan, Baguio City, and I am
fully dependent upon my parents for support.
2. I am not gainfully employed since I am still a high school
student.
3. I am executing this affidavit of non-employment for the purpose
of submitting this affidavit in support of my petition for change of
gender before the Local Civil Registrar of La Trinidad, Benguet
and for all legal intents it may serve.
IN WITNESS, I have hereunto set my hand this 15th day of
March 2016, in the City of Baguio, Philippines.
