AFFIDAVIT
I, ARIE JOY PAULI S. GUERRERO, of legal age, single,
Filipino, and with residence and postal address at Brgy. Ricudo, Sinait,
Ilocos Sur, on oath, states:
1. I am a Registered Medical Technologist and has been employed in
Corpuz Clinic and Hospital from October 1, 2012 up to March 31,
2013.
2. While working with the said hospital, I was not given an
employment contract.
3. To attest my employment, I hereby attached a photocopy of my
certificate of employment issued by the said hospital noted by Dr.
Martin V. Corpuz with Tin # 115-628-455 and PTR # 1478194.
4. I am executing this affidavit to attest to the truth of the foregoing
statements for all legal intents and purposes that this may well
serve.
IN WITNESS, I have hereunto set my hand this 15 th day of March
