PERMANENT BASIS IN THE PRIMARY & SECONDARY HEALTH-CARE
DEPARTMENT._______

With reference to your application for the above post.


Important Note

a)

You are being called provisionally for interview based on the data provided by you on the
application form.

b)

Information provided by you in filled form will be cross checked with the original
documents before the interview and if found correct, you will be allowed to participate in the
interview.

c)

In case any false information or variation is observed between information provided on


the form and the original documents presented by you on the interview day, your candidature will
be rejected, as undertaken by you on the application form.

You are required to produce the following DOCUMENTS IN ORIGINAL at the time of
interview, failing which you will not be interviewed and your application / candidature will be
rejected:i).

Matriculation Certificate or in case of O Level, Equivalent Certificate from Inter Board


Committee of Chairmen, Islamabad showing Total / Obtained
Marks, Date of Birth. Or
School leaving Certificate showing Date of Birth.

ii).

Intermediate Certificate / A Level along with Equivalent Qualification from IBCC,


Islamabad, showing Total / Obtained Marks.

iii).

Degrees/ Diplomas / Detailed Marks Certificates of Relevant Academic Certificates duly


issued by Controller of Examinations showing Total / Obtained
Marks, CGPA / GPA or
Percentages of Marks.

iv).

Degree and Detailed marks of MBBS (First to Final Profs.) duly issued by Controller of
Examinations showing Total / Obtained Marks and Percentages or Equivalent Qualification
duly issued by Controller of Exam. In case detailed marks are not produced then

minimum passing marks will be considered.


v).

Postgraduate qualifications/Degree/Diploma in the subject recognized / registered with


concerned council.

vi).

Domicile Certificate of relevant District of Province of Punjab issued on or before the


Closing Date.

vii).

In case of Government Servant, the Departmental Permission Certificate (format available


on PPSC website) duly signed and stamped by the Competent Authority of concerned
Department.

viii).

Experience certificate (Specimen available on PPSC website) duly signed by the Competent
Authority.

ix).

Valid C.N.I.C. (To appear in interview only original CNIC issued by NADRA will be
accepted. No other ID document will be acceptable.)

x).

Two Passport Size Photographs. (If not scanned on the Online Application Form)

xi).

Disability Certificate, if affected in Column 20 of Online Application Form has been ticked.

xii).

Foreign qualification(s) must be recognized by HEC and declared Equivalent by the


Q.E.D.C of the concerned Department.

xiii).

Equivalence of the Qualifications / Degrees will determined by the Qualification Equivalence


Determination Committee (QEDC) of concerned Administrative Department.

xiv).

Valid P.M.D.C Registration Certificate showing all Medical/Dental Degrees / Postgraduate


Qualifications therein. Provisional Registration with P.M.D.C issued for house job only is
not acceptable.

xv).

Two photocopy sets of your all credentials.

xvi).

Original Treasury Receipt of fee deposited.

xvii).

No TA/DA will be paid in this regard.
