This truss was being dismantled

The 275m chimney is being constructed

by EPIL and executed by Paharpur
Cooling Towers Ltd (PCTL). It has
reached 269m and the removal of slip
form is in progress. During the
dismantling of south west main truss
with wire rope/pulley arrangements, the
insert plate in which the diversion pulley
was fixed got sheared due to load on the
wire rope. Subsequently the wire
rope/pulley arrangement hit the two
persons who were standing nearby. One
person got injury in the left leg and other
got injury in both the legs.

Immediate causes:
Overload either from unremoved weld by
gouging or rope struck in between pulleys
Root causes (if known at the time of reporting):
Shearing of insert plate in which the diversion
pulley was fixed.

What you can do?
1. The main truss should be totally isolated from the main beam to prevent unnecessary jerks while operating through
winch machine. 2. Extra support should be provided on the insert plates to prevent the shear in case of external load.

Key Message: Winch machine and remote operator coordination to be ensured without failure.
