Career Plan

I am presently working to become a Licensed Nursing Assistant and have also received training as a
Personal Care Attendant and Emergency Medical Responder. I plan to apply to UVM Medical Center
College to pursue a degree in Nursing.
The basic requirements of my goal includes: greater than 90 hours of nursing assistant theory, greater
than 30 hours of hands on skills laboratory, and greater than 80 hours of clinical on the job training
experience, greater than 40 hours of Emergency Medical Responder, and greater than 40 hours of
Personal Care Attendant curriculum training. Other requirements are training and certification in AHA
First Aid and CPR for Healthcare Providers. I must also complete several hours of instruction in medical
terminology and human biology with a concentration in anatomy and physiology. I will also pursue 3
college credits in Medical Terminology and 3 college credits in Human Biology through Community
College of Vermont. I plan to volunteer at least 10 hours in community service projects as one of my
requirements to complete my L.N.A. training. I will also assemble an extensive electronic Career Portfolio
including but not limited to Career Pursuit Information, Resume, Application Form, Letter of Introduction,
Letter of Resignation, 3 Letters of Recommendation, Samples of Work, State Skills Checklists for Nurse
Assisting and Personal Care Attendant, the five Skill Sheets for Emergency Medical Responder, my High
School Transcript, a Copy of My Drivers License or Vermont State I.D., a Copy of My Social Security
Card, a Copy of my Birth Certificate, a Copy of My Immunization Record, a Copy of my Physical, and a
letter stating I have been cleared to perform L.N.A. duties.
The requirements I have met include all of the above including participating in greater than 90 hours of
nursing assistant theory, greater than 30 hours of hands on skills laboratory, and greater than 80 hours of
clinical on the job training experience, greater than 40 hours of Emergency Medical Responder, and
greater than 40 hours of Personal Care Attendant curriculum training. Other requirements I have met

renewal date of 09/2017. I have completed several hours of instruction in medical terminology and
human biology with a concentration in anatomy and physiology. I have successfully completed the
required hours of instruction in medical terminology and human biology to receive college credit from the
Community College of Vermont and through our articulation agreements. I have volunteered greater than
10 hours of my time in community service projects. I have also assembled an extensive electronic Career

Medical Terminology Final, and my Human Biology Final that are scheduled for the last week in May.
I will have completed my goal by June 10, 2017.
