STEPS IN THE APPLICATION FOR CERTIFICATE OF PUBLIC

CONVENIENCE (CPC)

Basic Requirements:
** photocopies upon filing of application; originals to be presented during hearing

Verified Application Form (5 copies)


Valid Government-Issued Identification Card with photograph of Applicant
Operators Data Sheet with recent 2x2 photograph (2 copies)
Route Measured Capacity (RMC) Certification from DOTC
Evidence of Filipino Citizenship
For individual applicant:
-authenticated birth certificate from NSO
Note: verify if it is issued within the past 3 months prior to application

-valid Philippine passport


For corporation:
-articles of partnership/incorporation and by-laws for partnership or
corporation
Note: verify if it is the latest AoI and By-laws as filed before the SEC

-articles of cooperation and by-laws for cooperatives

Note: verify if it is the latest AoC and By-laws as filed before the SEC

6. Statement of Financial Capability Form


7. Evidence of Financial Capability Income Tax Return
Note: verify if it is the latest ITR copy as filed before the BIR

8. Certificate of Average daily balance from the bank from the past year up to
present
Note: verify if the current balance in bank, at least 1 month prior to application, correspond to
LTFRB requirement of Php 30,000 per unit for Public Utility Buses.

9. Certificate of Business Name issued by DTI


10.Certificate of registration (CR) and Official receipt (OR) of authorized units
with year model duly issued by LTO
11.5R photographs of unit/s subject of the application (front, back and sides of
the vehicles)

Note: Photo should include the front page of a newspaper to show the date when the picture
was taken

12.Location Map and Picture with dimension of garage

Note: Should include Transfer certificate of Title or Tax declaration (owner) or Notarized
Contract of Lease with specific garage area (lessee)

13.Proof of Publication of Notice of Hearing

Note: Affidavit of publication or copy of notice as published should be presented.

14.Additional requirements for corporation:


Board resolution / Secretarys Certificate of authorized representative
Certificate of Good Standing from SEC if incorporated for more than 2 years
Note: Validity of the CPC is 5 years.

STEP 1: Download Required Forms


Concerned LTFRB Division: None

Download the following Application either in the LTFRB website or get the
forms from LTFRB help Desk
Application Form Template;
Requirement Form;
Statement of Financial Capability Form; and
Processing Schedule Forms

STEP 2: Present Verified Application Form with corresponding Requirements

The Officer in charge will check completeness of application particularly


submitted documentary requirements;
A call number is given to the applicant

STEP 3: Encode and Verify Application

STEP 4: Preparation of POS to include fees

Once submission is completed and verification is cleared.

STEP 5: Settlement of payment

Receive POS and application documents and proceed to cashier to give POS
and settle payment.

STEP 6: Receive Application Documents and provide applicant with PSF

Proceed to TED (Window 6), present original OR and submit photocopy of OR


and application documents including accomplished PSF.
The PSF indicates the schedule when to come back to claim Notice of hearing

STEP 7: Receive PSF then return on scheduled date

Step 8: Application documents with financial evaluation forwarded to Legal Division

The application documents with financial evaluation report is forwarded to


Legal Division
The Legal Division receive, log case folder and stamp schedule of hearing on
the case folder.
The Legal Division prepare 5 copies of Notice of Hearing and forward to chief
of Legal Division
Sign 5 copies of Notice of Hearing

Officer-In-Charge: Ms. Imelda Castro; Ms. Judy Garcia and Atty. Gonzalo Go
Duration: 2 days
STEP 9: Release of Notices of Hearing

Proceed to Legal (Window 16), present PSF, retrieve Notice of Hearing and
come back on scheduled hearing date.
Notice of hearing should be published and copies forwarded to affected
parties. Proof of publication of the Notice of Hearing and originals of
documentary requirements shall be presented during the hearing.
Release of Notices of Hearing to recipients

STEP 10: Post Notice of Hearing in the LTFRB website

Upon receipt, the MID will post the notice of hearing in the LTFRB website and
provide advance information to the applicant and affected parties through
SMS

STEP 11: Attend Hearing

The applicant shall attend the hearing on scheduled date.


The Legal will conduct the hearing. One (1) resetting may be allowed in case
of incomplete documents only.

STEP 12: Await LTFRB Decision / Resolution

STEPS IN THE APPLICATION FOR CERTIFICATE OF PUBLIC


CONVENIENCE (CPC)

The applicant will receive the LTFRB Decision/Resolution relative to the


application via Registered Mail. Applicant is also notified via SMS.
