SUBJECT: Lost Materials in Speech Laboratory

Good day Sir!

Yesterday, March 5, 2012, Mrs. Britanico advised me to open the
Speech Laboratory and to instruct Mr. Compaero and Mrs. Ng on
how to use it.
When I opened the Speech Laboratory and switch the
electricity and main server, I found out there are missing on the
mixer. I discovered all the switch button cover ( Black color) and
even the audio jack are missing. Although we can still use the
Speech Laboratory without those missing parts, I test all the
newly headset and I was shock there are 4 new missing headset
monacorr on the first row and the old 30 pcs headset are only 16
pcs now so it means 14 pcs are missing. Last January 2012
before the PACUCOA visitation, Alvin Avila, Arnold Batol and I
checked and tested all parts and connectivity of Speech
Laboratory. We certainly sure that there are no missing
materials/parts and all are intact.
Hoping for your immediate action regarding this matter.
