RESERVATION AGREEMENT FOR CENTENNIAL CLUBHOUSE

In consideration for the Centennial Neighborhood Association allowing me the


exclusive use of the Centennial Clubhouse, equipment and furnishings, I, the
undersigned, agree as follows:

I understand that continued use of the Clubhouse after the day of rental,
which includes a 24-hour period from 6:00 AM to 6:00 AM the following
day, will constitute a breach of this agreement and will result in forfeiture
of my deposit.
I will make a payment in the amount of $100.00 for rental and a
refundable deposit in the amount of $175.00 to cover any damages. The
Association reserves the right to keep the deposit if any damages are
found during the post-rental walkthrough. All fees must be RECEIVED at
least one week before the rental date.
I further understand and agree that the refundable deposit for damages,
will be used to pay for cleaning costs and any and all damages resulting
to the Clubhouse, its contents, or any other portion of the property from
any actions of persons present at, or attending, or in any other way
related to my function. I understand that any charges made against my
refundable deposit for damages will be explained.

If the cost of repairs exceeds the amount of my deposit, I agree to pay


the Association the full cost of all repairs within ten (10) business days of
receipt of a written explanation of the damages and a bill from the
Association for such repairs. I agree that all deposits, fees and expenses
incurred by the Association as a result of the use of the Clubhouse under
this agreement shall be considered as an assessment and constitute a
lien against my property and shall be fully collectible, as such as
provided for in the Association's By-Laws.
I assume all responsibility, risks, liabilities and hazards incidental to the
activities applied for (including, but not limited to, the serving of
alcoholic beverages), and hereby release and forever discharge the
Association, its officers, directors, employees, agents and members,
past, present and future, from any and all claims, costs, causes of action
and liability for personal injury or death and damage to or destruction of
property arising from my use of the Clubhouse and its appurtenances.
I agree to indemnify and hold harmless the Association, Community
Management Associates, its officers, directors, employees, agents and
members, past, present and future, from any and all claims, costs,

causes of action and liability (including, but not limited to, attorneys
fees) for any injury, to either person or property, suffered by me, my
family members, employees, agents, servants, guests, invitees or any
member of the Association or any other person which arise from or are in
any way related to the above activity, whether or not caused by the
Association's negligence.
7. I assume all responsibility for the actions and behavior of all persons
present at, attending, or in any other way related to my function and
agree to be personally responsible for causing all such persons to comply
with the Association's Declaration, By-Laws and Rules and Regulations. I
acknowledge that violation thereof by any person present at, attending,
or in any other way related to my function, may, at the sole discretion of
the Association's Board of Directors, result in forfeiture of my refundable
damage deposit.
8. I understand that I'm being granted the exclusive use of the Clubhouse
for the time period described above subject to the right herein reserved
by the Association to enter the Clubhouse and terminate my use thereof
should the conduct of any person using the facility endanger the health,
safety or well being of any person or constitute a threat of damage to
Association Property.
9. I am at least eighteen (18) years of age and will be in attendance at my
function. I hereby agree and represent that the Clubhouse will be used
for lawful purposes only and that if any conduct at the function I'm
sponsoring violates federal, state or local laws or ordinances, my rights
to use the Clubhouse under this Agreement shall be terminated and the
Association shall have the right to take possession of the Clubhouse and
instruct my guests to leave the property.
10. I agree to clean the facilities after use.
11. I understand that this reservation allows for the use of the Clubhouse

and restrooms ONLY, and does not include the use of the exercise room,
swimming pool or pool deck area.
12. I have carefully read and understand this form and the attached General
Association Clubhouse Rules and Regulations and agree to be bound by
its terms.

GENERAL ASSOCIATION CLUBHOUSE RULES AND


REGULATIONS
1. A pre-event walkthrough with a representative of the Association will be

accomplished prior to a key being issued, and a post-event walkthrough must


be accomplished within 24-hours or prior to the next scheduled event,
whichever comes first. The key must be returned to the Association at the postevent walkthrough. Items left at the Clubhouse after final walk-through
become the property of the Neighborhood Association.
2. The pool cannot be reserved for use in conjunction with a Clubhouse

reservation.
3. Clubhouse usage includes the front porch and lawn, but does not include the

exercise room, pool or pool deck area. Exceptions may include certain
community-wide Association sponsored events.
4. No paint, tape or tacks are allowed on walls or ceilings. No helium balloons

are allowed inside the clubhouse as they tend to get caught in the ceiling
fans.
5. Clubhouse furniture is to remain inside, except folding chairs and tables, which

are permitted outside until the event is over, at which time they must be
returned to inside storage. Furniture may NOT be moved into other
areas of the Clubhouse, such as the storage rooms or kitchen. It
must remain in the open area of the Clubhouse.
6. One adult must be present for every eight (8) persons when the age of the

group averages 18 -years or younger.


7. A security guard must be hired for any group of thirty (30) or more if there is a

majority of members between the ages of 18 and 25.


8. At Association functions, alcoholic beverages are permitted on a "bring your

own" basis only and are only to be consumed by persons 21-years of age and
older. Under no circumstances will alcoholic beverages be served by the
Association.
9. Loud music must cease at 10:30 p.m. Sunday through Thursday, 12:30 A.M.

Friday and Saturday.


10. All events, parties, etc. held in the Clubhouse or pool must be over by 12:00

AM, Sunday through Thursday, and 1:00 AM on Friday and Saturday.


11. As part of the required clean up, all trash must be placed in the cans outside

the clubhouse. They are located in the fenced area on the left end of the
building. If they are full, any additional trash must be removed from the
premises.
12. The clubhouse has a security system which should be armed when you arrive.

If the alarm beeps when you open the door, you will need to disarm it by typing
in the code 2424. When you leave, once all windows and doors are closed,
push the On button and then depart the building, locking it securely behind

you.
