The inspection was in response to spot check by Division staff on May 3%, 2016 that noted a potential problem of loss of containment in the lined pond, Cell 1A, whichis an Environmental Protection Facility (EPF). As reminder the Division expressed concerts to Colorado Goldfields regarding available freeboard in October of 2015 in an inspection report, as well as aguin in April 2016 based on « March 30* inspection ‘The Division requested Colorado Goldfields monitor the situation and submit photo documentation of those actvities tothe Division. No documentation has been received by the Division regarding the requested monitoring
