EXHIBIT FEE:
The costs of exhibit space are as follows:
$500 Standard Booth- Booth, Company name on Annual Conference materials
$675 Prime Booth- Booth, Company name on Annual Conference materials, Company advertisement in
WVPTA newsletters, Meal Sponsor- your name and/or logo displayed on signage, and BONUS: Your
Choice of ONE of the following...1) Your company name/logo included in a Post-Conference Email
Blast to all Attendees 2) A 3-month ad on the WVPTA website 3) One complimentary round of golf
$2,500-2017 Partner- Preferred booth location, Company name on WVPTA 2017 event material (Annual
Conference, PT After Hours, Payer Forum, Winter Conference), Company advertisement and link to webpage
on WVPTA website & newsletters, Database marketing (access to over 600 WVPTA member mailing labels),
Two complimentary rounds of golf
Payment is due as soon as possible, but no later than March 31, 2017.
Checks should be made payable to the WVPTA and should be mailed to 650 Main Street, Barboursville, WV 25504.

7:30 am - 6:00 pm
7:00 am - Noon
EXHIBIT VISITATION: The Exhibit Area will be located on the Second Floor in the Birch Room and Foyer, adjacent
to the Stonewall Ballroom where lectures are held. Complimentary breakfast, and refreshment breaks will be served each
day near the exhibit area for both exhibitors and registrants to ensure booth visitation. Please note: Registrants will be given
a door prize card to be signed by booth representatives and valuable door prizes will be awarded. In addition, a silent
auction will take place throughout the conference in the exhibit area. Exhibitors are welcome to enjoy the breakfast and
refreshment buffets.
