AFFIDAVIT OF SUPPORT AND CONSENT TO TRAVEL

I, EDUARDO L. MARTEJA, of legal age, Filipino, married, with residence at

and presently residing at 3949 Guinto St. Purok Jasmin, Pinagbuhatan Pasig
City, after having been duly sworn to in accordance with law, depose and state:
1. That I am the biological father of EDISON M. MARTEJA
who was born on 05 June 1999 in Pasig City.
2. That said child will travel to Hongkong for vacation;
3. That said child will be leaving on 29 January 2016 and will
stay in the said country for a period of three (3) days and will be
taken cared of by his siblings who are all of legal age, to wit: Mary
Edaline M. Marteja, Esmeralda M. Marteja, and Marrion M. Marteja;
4. That I cannot personally travel with my child due to urgent
work in the Philippines;
5. That I am giving my full consent for the minor child to
travel abroad to be accompanied by his siblings who are all of legal
age, to wit: Mary Edaline M. Marteja, Esmeralda M. Marteja, and
Marrion M. Marteja;
6. That I personally guarantee the financial support of the
child in the course of his travel and stay outside the Philippines and
further guarantee that he will not in any manner become a burden
to the Philippine and Hongkong governments.
7. That I am executing this affidavit in connection with my
child's application for a passport at the Department of Foreign
Affairs and/or travel clearance from the Department of Social
Welfare and Development for his travel abroad.
