I, JUAN DELA CRUZ, of legal age, Filipino, single and a
resident of Digos City, after having been duly sworn to in
accordance with law, do hereby depose and say:
1. That I am the same JUAN DELA CRUZ who caused
the execution of this affidavit;
2. That I am the secretary of Atty. Maria Thaisa D.
Justol;
3. That on January 9, 2017, I served copies of the
Petition for Review in NPS Docket No. XI-04-INV16B-00106;
4. That due to the distance between the office of Atty.
Maria Thaisa Justol and the Office of the Secretary,
National Prosecution Service, Manila, copies of the
above-mentioned Petition for Review were sent via
registered mail;
5. That this affidavit was executed to attest to the truth
of the foregoing facts and for whatever legal purpose
this may served.
