    Subject: REQUEST FOR ISSUANCE OF EXAMINATION FORM.

    R/sir,
    It is humbly requested in your honour that due to some family
    problems I was not able to attend the class whole last year ,after coming to my
    knowledge that examinations forms are being filled I make it to concern office but
    they told me to give an application in your honour and after grant of that
    application they will issue me examination form.
    It is requested you to please grant me permission to fill examination form so I
    would be able to give examination.
    I shall be thankful for your this act of kindness.
