The Use of Pre/Post Hemodialysis Evaluation Tool as a Template to Enhance Documentation

Problem:
The community Living Center needs to meet the challenge in Long TermCareSurvey report
that evidence is needed to support documentation on assessing, evaluating patient receiving
hemodialysis before transporting patient to dialysis unit and after patient returned to Community
Living Center (CLC) from dialysis unit.
Evidence:
Patient needs to be assessed, evaluated before and after receiving hemodialysis, and after
performing assessment, evaluation of patient needs to be documented.
Strategy:
Hemodialysis committee was formed. Pre/Post hemodialysis evaluation tool as a template for
documentation on assessing, evaluating patient before transporting patient to dialysis unit and
after returning patient to CLC from dialysis unit was initiated and completed. The evaluation
tool is used for assessing, evaluating patients vital signs, pain, weight, level of consciousness,
auscultation of lungs, peripheral edema, vascular access present/patent.
Practice Change:
The practice of documentation on assessing, evaluating patient before and after receiving
hemodialysis by documenting in pre/post hemodialysis evaluation tool was established.
It was implemented throughout the all units in CLC.
Evaluation:
The rate of compliance on documenting pre and post hemodialysis evaluation tool were tracked
on all units in CLC during the following year.
Results:
Overall, 92% compliance rate was achieved on documenting in pre/post hemodialysis evaluation
tool before transporting patient to dialysis unit and after returning patient to CLC from dialysis
unit for the following year after implementation.
Recommendation:
The result shows the pre/post hemodialysis evaluation tool is very important and useful tool for
increasing compliance on documentation in assessing, evaluating patient before transporting
patient to dialysis unit and after returning patient to CLC from dialysis unit for receiving
hemodialysis.
Lesson Learned:
Implementing the pre/post hemodialysis evaluation tool is very effective in compliance and to
standardize practice on documentation.
