FORM B-3. PRE-VISA AND DS160 GUIDELINES

STEP 1
MRV
Payment

1. Please pay the MRV (Embassy) FEE to any BPI Branch.

a. Please print the deposit slip for J1 Visa Application by following these steps:
i. Click link: http://www.ustraveldocs.com/ph/ph-niv-
paymentinfo.asp#feepaymentoptions
ii. Click Deposit Slip - $160
iii. Make sure you print it the day before or during the day you intend to pay
for it has an expiration date.
iv. Make sure that the amount and the receipt number is seen. Otherwise,
BPI will not accept your deposit slip.
b. Bring a copy of your passport with you.
c. Some BPI branches have the automated machine. Go to the machine, choose US
VISA Payments, NON IMMIGRANT VISA, select J1 and enter your passport
number. If they do not have an automated machine, please proceed to the teller.
d. You will need to pay this in Philippine peso, please bring the amount stated on
the deposit slip ($160).

REMINDER: Please keep the original deposit slip and the printed slip for your US Embassy
interview.

2. After payment, please scan the deposit slip and send it to your program Advisor.

3. Please fill up the Questions for FORM F. VISA GUIDE QUESTIONS that was given to you
by your program advisor thoroughly as this will serve as your guide in answering the
Online VISA application (DS-160)

4. Wait for your program advisor to send you your access to the DS-160 (Online VISA
application) form. Example:
a. DS-160 Application ID Number: AA00ABCQW1
b. Answer to the Security Question: LOPEZ
c. SEVIS Number: N0005842869***
d. PROGRAM Number: P-4-12345***

*** The SEVIS NUMBER and the PROGRAM NUMBER will not be given to you if you
have not paid your remaining balance.

STEP 4
Scan 2x2 ID
Photo

5. The DS-160 form requires you to submit a 2x2 ID photo. The photo should be:
a. Colored and in a white/off-white background
b. Taken within the last 6 months
c. Full-face view directly facing the camera
d. With a neutral facial expression and both eyes open
Should you wish to test your photo ID, you may log on to: http://ceac.state.gov/genniv
and you will find a TEST PHOTO ID button.
6. After having a photo ID, scan it and save it on file.



7. Go to http://ceac.state.gov/genniv.

8. Select PHILIPPINES, MANILA from the dropdown menu.

9. Click RETRIEVE AN APPLICATION from the choices.
10. You will be asked to input your DS-160 APPLICATION ID NUMBER (format:
AA00ABCQW1).
11. You will also be asked to type in the FIRST 5 Letters of your LAST NAME, YEAR OF
BIRTH, and the answer to the SECURITY QUESTION.

Use the one given to you by your program advisor in STEP 3.

12. After successfully retrieving your DS-160 online form, please fill the questions out.
13. Use the Questions for DS-160 form (FORM P) you have answered in STEP 2 as your
guide to save time.

REMINDER: Please do not spend too much time answering the form. It expires after 20
minutes.

14. After the Security Questions, you will be directed to the SEVIS and PROGRAM
Information. Please put the SEVIS and PROGRAM information given to you by your
program Advisor in STEP 3. The format:
a. SEVIS NUMBER N0005842869
b. PROGRAM NUMBER P-4-12345

REMINDER: If you have not settled your balance, you will not be able to get your SEVIS and
PROGRAM numbers hence, you will not be able accomplish your DS-160 Online Application.

15. Please upload the photo you scanned and saved in STEP 4.

16. After uploading your photo, you will be asked to review the form.
DO NOT REVIEW AND CONFIRM IT YET. Your Program Advisor will need to review it
first. Click Exit on the upper right side of the screen.
17. Confirm Exit by clicking EXIT on the pop up menu.
18. Notify your Program Advisor that you have accomplished the form. You will not be
allowed to go to the office for the Pre-Visa Orientation without accomplishing the DS-
160 until the REVIEW PART OF THE APPLICATION.
