Application Letter
Personal Data Sheet (PDS) with latest 22 I.D. picture
Certified true copy of diploma
Authenticated Transcript of Records
Certificate of eligibility/board exam result,if applicable
Certificates of relevant training, seminars and awards/recognition, if any
Performance rating for the last two (2) rating periods, if applicable
Certificate of previous employment
Certification of no pending administrative charge, if applicable; and
NBI clearance, if applicable
