JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, AGNES B. RAMALES, of legal age, Filipino citizen, and a
resident of Barangay Sta. Maria, Camiling, Tarlac, and KAREN G.
SOLIVIO, of legal age, Filipino citizen, and a resident of Barangay
Anoling 1st, Camiling, Tarlac, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:
1.

We both personally know BETSAIDA DE JESUS

SALCEDO (Betsaida, for brevity), born on February 15, 1968 to
Spouses Dionicio Salcedo and Juanita De Jesis in San Clemente,
Tarlac, Philippines and who got married to Reynaldo B. Rosario,
hence, she is also known as MRS. BETSAIDA SALCEDO
ROSARIO;

2.

Betsaida is a bonafide member of the Social Security

System (SSS) with SSS No. 02-2947785-4;

3.

Lately, when Betsaida went to the SSS Camiling, Tarlac to

continue her contributions, she was required to submit copies of her
Certificate of Birth and Marriage Contract;

4.

After submitting the required documents to the SSS and

upon processing, it was discovered that the recorded first name of
Betsaida appearing in her SSS Self-Employed Data Record (SS RS1) is BEBAIDA, hence, the discrepancy which might have been due
to typographical error;

5.

We are not related to Betsaida by affinity or

consanguinity, except that of being his neighbors and barangay
mates;

6.

We are executing this Affidavit to attest to the fact that

the names BETSAIDA J. SALCEDO, as recorded in her abovementioned Certificate of Birth and Marriage Contract, ROSARIO,
BEBAIDA SALCEDO, as recorded in her SSS Self-Employed Data
Record (SS RS-1), and MRS. BETSAIDA SALCEDO ROSARIO,
refer to one and the same persons, and that is our neighbor and
barangay mate;

Finally, we are executing this Affidavit to attest to the

truth of the foregoing statements and for all legal intents and
purposes it may serve.
