No nasal obstruction
Able to identify smell
Visual acuity of 20/50 for right eye,
and 20/70 for left eye
Visual fields intact
Pupils round and constrict from 4 mm
to 3 mm
Direct and consensual reactions to
light are positive
Pupillary constriction observed
Accommodation and convergence are
intact
Extra ocular muscle movements are
conjugate
Nystagmus is absent
temporal and masseter strength is
intact
Pain sensation is detected bilaterally
Light touch is detected bilaterally
Corneal reflex is intact
Facial movement and expression are
intact and symmetrical
Sound was heard within 1 feet for
both ears by whisper test
Soft palate rose
Uvula remained midline
Intact gag reflex
No atrophy or fasciculation in
trapezius muscle
Trapezii strength intact
Tongue is symmetrical with no atrophy
or fasciculation and remained midline
upon protrusion
There is adequate articulation of
words
