1. In order to apply for a new TLC license you will need to submit all required items for your license type as listed on this checklist.
2. You must provide the all required documents as well as clear and legible copies .
3. Documents will not be accepted if they are altered or written in pencil.
ALL REQUIREMENTS MUST BE MET WITHIN THE NINETY (90) CALENDAR DAYS FROM THE DATE YOU
SUBMIT YOUR APPLICATION TO THE TLC OR YOUR APPLICATION WILL BE DENIED.
APPLICANTS ARE RESPONSIBLE FOR REVIEWING ALL REQUIREMENTS LISTED ON THIS CHECKLIST.

Sex Trafficking Awareness Training Must view the required video located on the TLCs website pertaining to the unlawful use
of a TLC licensed vehicle for the purposes of promoting prostitution. This must be done prior to the submission of this

Rules

Fitness Rules If you have a history of TLC violations, DMV violations, accidents, or have had a previous TLC license revoked,
or TLC application denied, your application for a new TLC license may be rejected. You should not submit an application if your
TLC or DMV history matches the description listed on the Fitness Rules Notice. For more information, please review the Fitness
Rules Notice available at: www.nyc.gov/drivernewinfo.

sealed and cannot be dated more than ninety (90) days at the time you submit your application to the TLC.

Your DMV license cannot be suspended, revoked, conditional, or restricted.

The picture on your DMV license should resemble your current appearance. Copies of your DMV license
and a temporary license (interim) will not be accepted if you do not have your original DMV license.

To the TLC, NYS DMV Traffic Parking Violations Bureau (TVB), Department of Finance (DOF) Parking Violations and DOF Red
Light Bureau. If any outstanding judgments are discovered (i.e., unpaid tickets), your application will not be processed.
