SAVE A COPY OF THIS FORM ELECTRONICALLY. Since it is to be used as the Form 1-A, keeping an electronic copy
will allow you to do your Adds and Deletes to your program without retyping the entire form.
ALL FORMS MUST BE TYPED. Handwritten forms will be returned. You must submit the Form 1, Program of Study,
within six weeks after the beginning of the semester in which you are taking your 15th graduate credit hour in consultation
with your advisor and selected committee. Complete :

If you are taking courses
at the Missouri S & T campus, check "On Campus"'
at the EEC in St. Louis, check "EEC";
at Ft. Leonard Wood, check EOAC

List the degree program you are seeking (not the department name)
List all undergraduate and graduate college training and the schools attended & year degree awarded if
applicable. E.g. Btech, Mech. Engr., Mumbai Univ., India, 4/05; MS, Sys. Engr., UMR, 12/07
Program Status - indicate if this is a new program(Form 1) or a revised program (Form 1-A).
For NEW program (Form 1): You must list all courses that will be applied toward the degree. List 3000 level first, transfer
courses next, 4000/5000 lec, 6000 lec, then Research/Special Problems/Seminar (Res/Sp Prob/Sem)
=5099/6099/4000/5000/6000/5010/6010. For every course you list, indicate:
Course department and number (e.g. Math 5325)
Official course title as listed in Graduate Catalog. If Special Topics (4001/5001/6001), indicate course name not
"Special Topics" as course name
Semester and year course has been or will be completed--SS=summer session; SP=Spring semester; FS=Fall
semester
Indicate credit hours under the appropriate box (no check marks numbers) e.g. 3
1000-2000 numbered courses are NOT allowed on a graduate program.
3000-level lecture courses (old numbers 200 level)
9 credit hours minimum for non-thesis seeking students
Maximum 4 credit hours special problems (4000/5000/6000) and/or seminar (5010/6010) for non-thesis
Indicate Total Credit Hours
30 credit hours minimum for most MS degrees (see department for requirements)
Some departments require 33-36 credit hours (see department for requirements)
MBA requires 36 credit hours
2nd MS requires 24 credit hours minimum
A copy of transcript showing 1st Masters degree was awarded must be attached to Form 1 upon
submission
NOTE: As this is an Excel file, additional rows may be added as needed for more courses or an additional committee
member.
For REVISED program (1-A):
Add(A)/Delete(D) - use for changing program but not for new programs
List ALL courses on your program (must match exact to your original Form 1) and then do the following:
Indicate capital A if adding course to current program
Indicate capital D if deleting course from current program
Indicate Total Credit Hours
NOTE: As this is an Excel file, additional rows may be added as needed for more courses or an additional committee
member.
Change in Committee:
Use only as change in program; not for new programs.
Indicate who is being replaced and who is replacing the former member--full names not just Dr. Smith
Reason for Change:
Short reason why you are changing committee member(s)
All committee members (including member(s) being removed) must sign change form. If committee member has
left the University, an email with his approval can be attached to the form.
Signatures
Student must sign and date form; advisor & committee must sign new program forms and any change forms
submitted
Type committee members names on line with signature
Advisor must be member of Graduate Faculty (gradfac.mst.edu/graduate faculty/membership/gradfaclist/)
Thesis student must have advisor & 2 committee members minimum. Advisor and at least 1 committee member
must be a member of Graduate Faculty
If a committee member is not on Graduate Faculty then a resume indicating educational background for that
committee member must accompany this form (whether they are on or off campus).
Non-thesis student has only advisor as committee
Department chair or designee must sign
Vice Provost for Graduate Studies (VPGS) or designee must sign
Additional Information
In addition to meeting Missouri S & T requirements, the plan must adhere to all department regulations
Once the plan receives departmental approval, submit the original typed and signed copy to the Office of Graduate
Studies. Once approved by VPGS, the original is sent to the Registrar's Office for student's permanent file, one copy is
sent to the department and one copy is kept in the Office of Graduate Studies.
For 2nd MS a transcript MUST be attached to the Form 1 showing 1st Masters was awarded.
