RELEASE, WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, TERESITA V. URBANO, of legal age, Filipino, married and

a resident of Purok Dama de Noche, Brgy. Maunlad, Puerto
Princesa City, Palawan, Philippines, in my own free will, and for a
valuable consideration, hereby declare and manifest:

1. That I am the mother of JEVIE V. URBANO, who was

employed with MITHRAS SECURITY AND
INVESTIGATION, INC.;

2. That he voluntarily resigned from his employment

effective October 16, 2015;

3. That in consideration with his separation from his

former employment with MITHRAS SECURITY AND INVESTIGATION,
INC., for valuable consideration in the amount of
____________________________, Philippine Currency, by these
present, I hereby release, waive and discharge MITHRAS
SECURITY AND INVESTIGATION, INC., its officers, directors,
representatives or employees from any action for sums of money
or other obligations arising from my sons previous employment
with MITHRAS, INC., I for and in behalf of my son, acknowledge
that I have received all amounts that are now or in the future may
be due from MITHRAS, INC.;

4. That I admit that I have no cause of action whatsoever,

whether criminal, civil or administrative, against MITHRAS, INC.,
its officers, agents or representatives with respect to any matters
relating to his former employment with MITHRAS, INC.
