Corporate Safety & Health Committee:
Corporate Safety and Health Committee was established in Dec. 1998.
The Chairman & Deputy Chairman was appointed by the BOD.
Safety & Health Officer is acting as the Secretary & meeting will be once
in 3 month.
Regional / WTP Safety & Health Committee:
There are 6 subsidiary Safety and Health Committee by Regional
Representative Segregation as follow:
Management Representative : Executive and above
Employee Representative : Operator/Technician and below
Frequency of meeting: At least once in every 2 months.
Each Regional / WTP SHC will meets and pending issues will be solved in
the Corporate SHC meeting end of third month.

Four (4) WTPs is identified as a Major Hazard (MHI) Plant since
the Chlorine storage capacity exceeds 10 MT.
1. Bukit Nanas WTP

CIMAH Report above WTPs was prepared by Competent Person
and has been submitted to Major Hazard Dept. of DOSH
Putrajaya in March 2010 (3rd Revision)
Information to Public was carried out / presented at respective
Local Council (Majlis Daerah Tempatan) meeting.
Emergency Response Plan established and tested annually by
conducting drill i.e Chemical Leaks or Fire.
Formation of the Chlorine Handling Team, First Aider and
Emergency Response Team and Chlorine Task Force.

Emergency Response Brief
Crisis Management Group (CMG) and Emergency Response Team (ERT)
have been formed comprises of various discipline and were put in
standby and rotation basis.
Trained regularly, in-house or at the manufacturer premises.
In the event of crisis, the crisis will be controlled from the Crisis
Command Centre which has all the facilities i.e. telephone, fax, WTPs
documents and etc.
CMG at HQ level is headed by Chief Operating Officer as Chairman of
Crisis Management Committee and the Safety and Health Officer as the
Advisor.
On-site CMG is headed by the General Manager or Regional Manager or
Plant Manager (depend on the level of crisis) as the Emergency Manager.
Procedure were established in Crisis Management Plan and WTP
Emergency Response Plan.

1. GREEN Will cause minimal interruption to the operation of a facility which could be rectified or remedied within 24 hours and
treated water loss of then 100 ML
The incident can be immediately brought under control without involving outside assistance
Does not attract media attention or public scrutiny
Its severity will not affect the safety of the public or the environment
Minor IT problem
2. YELLOW Will cause interruption to the operation of the WTP/Dams which could be rectified or remedied within 24 hours and
treated water loss of more than 100 ML but less than 300 ML
Cannot be immediately brought under control under Code Green
Has a potential to attract media attention and public scrutiny
Its severity will have an adverse effect on the safety of the public or environment
3. RED Will cause interruption to the operation of the WTP/Dams which could be rectified or remedied within 72 hours and
treated water loss of more than 300 ML
Poses an actual or potential threat to the long term ability of the company to do business due to its impact on PNSBs
operability, image and profitable
Has a potential to attract media attention and public scrutiny
Cannot be brought under control by Code Yellow

Accident Reporting & Investigation
Procedure established Accident Reporting and Investigation.
Currently the procedure is in the process of updating due to new
regulations released.
As at today only accident that categorized as First Aid case are
recorded.
Anybody can notify an accident to the Management or HSQM
Section but the Immediate Superior of the victim must forward a
report within 24 hours and DOSH / Authority.
Within 7 days, an investigation report of the accident must be
forwarded to the Management & state DOSH
Safety & Health Officer with the assistance of Accident
Investigation Team (AIT) is responsible to carry out the accident
investigation.
