This document provides information for teachers and staff that will be assisting teachers in
administering SAGE tests. The SAGE Summative test window is March 22 May 20, 2016 for all tests.
Throughout the Testing Window
Be familiar with the Standard Test Administration and Testing Ethics Manual, Jan 7, 2016 (especially
the references to Board Rule R277-404) http://www.rules.utah.gov/publicat/code/r277/r277-404.htm
Make sure all faculty and staff receive Ethics Training and sign the Ethics Policy Sign-off annually.
Be familiar with the SAGE Spring Summative Test Administration Manual 2015-2016.
Be familiar with the 2015-2016 Utah Accommodations and Participation Policy.
Send a copy of the Parental Exclusion (Opt-Out) forms to Erin Wright in Student Assessment as soon
as received.
a. Mark the Parental Exclusion special code in TIDE.
b. Inform Erin Wright if the student has started any of the tests so the tests can be invalidated.
Before Testing Begins (no later than March 22, 2016)
Teachers and Test Administrators
Work with your specialists, special education teachers, English as a second language teachers, and
STS/LMETS to identify the accommodations identified on your students IEP, LEP or 504. Your
STS/LMETS will enter this data in TIDE.
Consider having students practice by using the SAGE Training Test site via the Secure Browser before
taking their live test.
Secondary Math teachers should have their students try the Secondary Math Training Test
questions 1 and 2 which demonstrate a new haptic-based geometry proof.
During the Test Window March 22 May 20, 2016
Teacher and/or Test Administrator
Follow the USOE Standard Test Administration and Testing Ethics Policy.
Receive the SAGE Testing Logs and Test Tickets for each test from your STS/LMETS 1-2 days before
your testing begins. File both in a folder and keep it in a secure place in the lab or classroom where
students will be tested. There should be a Test Ticket for each student on your roster.
Assign student seating in the lab, thoughtfully and strategically.
Hand out Test Tickets and collect them at the end of the session.
Help students log in.
Administer and actively proctor the test.
If a student is absent, mark Absent on the SAGE Testing Log. Entering absent as a locks the student
out of their tests. (The Absent special code will be marked after the window is closed.)
Check that students have completed the whole test before they are allowed to submit the test, then
verify that they have submitted their test.
Keep track of students who are absent, have started, or have completed the SAGE test using the
SAGE Testing Log.
Document any problems that occur during testing (e.g. fire drill, network problem, SAGE down) on the
SAGE Testing Log.
Inform your STS/LMETS immediately after your classes complete testing and give them your SAGE
Testing Logs.
After Testing Ends on May 20, 2016
Teacher and/or Test Administrator
Work with your STS/LMETS to verify that all students in your class have completed tests, tests have
been submitted, and all special codes and accommodations have been entered in TIDE as soon as
your class is finished testing.
Enter a non-participation special code in TIDE for students who did not complete their SAGE tests.
