PETITION

PETITIONER, ___________, through the undersigned counsel and unto

this Honorable Court, most respectfully states that:

1. Petitioner is of legal age, Filipino, ______, and a resident of

_______________________________________.

2. On May ___________ petitioner gave birth to a child named ________________

with the birth certificate stating that the father of said child is one named
_____________________ who is not married to her at the time of the birth of the
child. The Certificate of Live Birth of petitioners son and affidavit of
acknowledgment/admission of paternity are attached as _____________;

3. About _________ thereafter, the whirlwind romance between the petitioner

and the childs father turned ugly, thereby leaving the herein petitioner with
the sole obligation to rear the child, and since then, petitioner had not heard
from said father;

4. Petitioner has been exclusively raising the child, having custody thereof, and
had been supporting the child from the time the father abandoned them and
until now;

5. Judicially confirming the parental authority of herein petitioner to her is for

the best interest of her child;

6. Since petitioner has settled in __, she intends to bring her child in said
country and petitioner even went home to the Philippines to look for her
sons father in order for her to obtain from him parental consent to travel;
7. One of the requirements of the Embassy of France is for herein petitioner to
obtain a consent from the father of the child to allow her to bring the child
with her to France, but despite earnest efforts to locate the father, the same
proved futile. Attached herein is a copy of the police blotter reporting their
failure to locate the said father as ________;

8. Due to the absence of the father and petitioners failure to obtain consent
from the father of her child, the Embassy of France instead required herein
petitioner to obtain a judgment granting to her the sole parental authority of
the child;

9. Under Art. 363 of the New Civil Code, the welfare of the child is of paramount
consideration in all questions regarding the care, custody, education and
property of the child, and granting the herein petition is for the welfare of
petitioners minor child;

10. Thus, this petition for judicial confirmation of sole parental authority
over her son ____________.

PRAYER
WHEREFORE, premises considered, it is most respectfully prayed
from this Honorable Court that after hearing, judgment be rendered in favor
of petitioner, granting:

1. The sole parental authority over her minor child _____________;

2. Authority to bring her child into the country of ____________;

Other reliefs that are just and equitable under the premises are
likewise prayed for.
