Two fire hydrants in the engine room with frozen valves and outlet caps. A hammer was needed
to free the outlet caps and a large tool was needed to free the valve wheel.Two hose stations in
the engine room without spanner wrenches.

PSCO observed that the vessel was unable to provide proof of the annual security exercise.
The fire control plan does not show an additional general, engine, and chemical material
storage room located within the sewage room.
Passage plan failed to use appropriate large scale charts.No evidence to indicate manual testing
of auto pilot as per STCW

Navigating officers fixing vessels position in coastal waters at 60 minute intervals for previous
voyage. SMS procedure requires fixing intervals of not more than 15 minutes in coastal
waters.Significant number of instances where bridge officers have fixed the vessels position
infrequently and using only one means of navigation (GPS). During Jomard transit GPS positions
hourly whilst vessel is 1.3' from navigation hazard. Chart A388 vessel is 2.2 from Nav. Hazard
and GPS positions 1h 32m apart.
Lifeboats (P/S) fitted with FPDs. FPD shackles Fwd. & Aft on lifeboat (P) and Aft FPD shackle on
lifeboat (S) seized
No evidence that the cargo operation has been monitored, no records of observed draft made
for the various stages of loading
