Symptoms
History: As per record, the patient had been having consult since 2010 and was admitted in
NCMH in July 2012. She was discharged improved with uncontrolled medication. A few minutes
prior to admission she was seen from the Emergency Room roaming around topless, crying with
hand shaking spells and filthy. Hence this she was admitted.

Evaluation:
The first and most important phase of a nurse-patient interaction is the Orientation
Phase. The goals of this phase are to establish a nurse-patient relationship, build trust and
rapport, and set parameters and goals for the student nurse and patient, and also to set
limitations and boundaries in the relationship. It was very challenging to talk and interact
with a psychiatric patient, as there are limitations to what you can say and do. My patient
is already a functional so I dont have any hard time to talk with her because shes
usually the one that initiates a topic. She has a lot of question and really shows
willingness to learn. Based on what I have observed sometimes she just blankly stares at
one place but can be able to get her attention quickly. I am very fortunate to have a
patient like her because she really does want to learn new things and she is very
cooperative.
