This Deed of Donation made and executed in Quezon City, Philippines by and

TECHNOLOGY, an institution duly organized and existing under and by virtue of
Executive Order No. 269 dated 12 January 2004, with principal office address at C.P.
Garcia Avenue, U.P. Diliman, Quezon City, represented herein by its Chairman, RAY
Principal, DAGUPAN CITY NATIONAL HIGH SCHOOL

law with principal office address at Tapuac, Dagupan City, Pangasinan, represented
herein by its Principal, THEODORA NABOR, hereinafter referred to as DONEE;

WHEREAS, the DONOR is the absolute owner of the following personal

property described as follows:

Twenty (20) Workstation Computers (consisting of tower case CPU, monitor,

keyboard, mouse, headset and UPS) with pre-installed Operating System &
Office Productivity Tools software
One (1) Server (consisting of tower case CPU, monitor, keyboard, mouse,
headset and UPS) with pre-installed Operating System & Office Productivity
Tools Software
One (1) Wireless LAN Router
Commissioner, H.C.D.G./ ICT4BE Program Director

Twenty-one (21) Wireless LAN Adapters
One (1) Three-in-One Printer
One (1) Multimedia Projector
CONSUELO S. PEREZ

Two (2) Airconditioning Units

One (1) Signage/Marker of the Laboratory

WHEREAS, the DONEE represents that it is an institution duly organized and

validly existing under the laws of the Republic of the Philippines and has the requisite
corporate authority to accept donations and to enter into, perform and execute this Deed
of Donation and Acceptance.
Witnesses:

WHEREAS, the DONOR is donating the aforementioned property subject to the

following Terms and Conditions:
iSchools Project Coordinator, ISPSC

1. The purpose for which the donation was approved and granted by the DONOR
EVANGELINE C. REYES

shall not be changed, modified or altered without the prior written consent of
the DONOR which will not be unreasonably withheld by the DONOR.
2. The DONEE shall submit a quarterly progress and financial reports and
programme development details to the DONOR on the use of the above
enumerated properties and donation.

 3. The DONEE shall not modify the configuration of the computers unless CICT-
HCDG iSchools Project Management Office issued a written certification to the
DONEE authorizing the said organization.
4. If the DONEE commits a breach of the conditions provided in the sections above,
the DONOR or its assigns, successors-interest, may take such action, including
the revocation of the donation, in order to protect its interest.
5. Upon the revocation of the donation, the DONEE shall forthwith return to the
DONOR within 30 days the donated items above enumerated without prejudice
to damages.
6. This Deed of Donation is executed to effect the agreement of the parties and the
conveyance in a Memorandum of Agreement between the DONOR and
DONEE dated ________________ herewith attached. In case of non-compliance
or violation of the Agreement by herein DONEE, the facilities and items
conveyed shall revert back to the DONOR.
7. The DONEE shall commit to the sustained operations of the iSchools Learning
Resource Center and shall use the computer hardware units, network
equipment, and electronic peripherals donated by the DONOR exclusively for
the operations of the iSchools LRC.
8. The parties hereto mutually agree that this donation in favor of the DONEE does
not in any way bind the DONOR to grant future requests for donation by the
DONEE.

NOW THEREFORE, for and in consideration of the performance and observance of the
above obligations, the said DONOR hereby cedes, transfers and conveys by way of
Donation, unto said DONEE the personal property above-described free from all liens
and encumbrances.

ACCEPTANCE

The DONEE hereby ACCEPTS the foregoing donation of the above-described

property for with its sincerest appreciation and gratitude for the kindness and
liberality of the DONOR.

13. Upon the revocation of the donation, the DONEE shall forthwith return to the
DONOR within 30 days the donated items above enumerated without prejudice
to damages.

TECHNOLOGY, an institution duly organized and existing under and by virtue of

La Union, represented herein by its Principal, MYRNA LIGAS, hereinafter referred to

as DONEE;

WHEREAS, the DONOR is the absolute owner of the following personal

property described as follows:

Twenty (20) Workstation Computers (consisting of tower case CPU, monitor,

keyboard, mouse, headset and UPS) with pre-installed Operating System &
Office Productivity Tools software
One (1) Server (consisting of tower case CPU, monitor, keyboard, mouse,
headset and UPS) with pre-installed Operating System & Office Productivity
Tools Software
One (1) Wireless LAN Router
Commissioner, H.C.D.G./ ICT4BE Program Director

Twenty-one (21) Wireless LAN Adapters
One (1) Three-in-One Printer
One (1) Multimedia Projector

Two (2) Airconditioning Units

One (1) Signage/Marker of the Laboratory

WHEREAS, the DONEE represents that it is an institution duly organized and

validly existing under the laws of the Republic of the Philippines and has the requisite
corporate authority to accept donations and to enter into, perform and execute this Deed
of Donation and Acceptance.
Witnesses:

WHEREAS, the DONOR is donating the aforementioned property subject to the

following Terms and Conditions:

73. The purpose for which the donation was approved and granted by the DONOR
EVANGELINE C. REYES

shall not be changed, modified or altered without the prior written consent of
the DONOR which will not be unreasonably withheld by the DONOR.
74. The DONEE shall submit a quarterly progress and financial reports and
programme development details to the DONOR on the use of the above
enumerated properties and donation.

 75. The DONEE shall not modify the configuration of the computers unless CICT-
HCDG iSchools Project Management Office issued a written certification to the
DONEE authorizing the said organization.
76. If the DONEE commits a breach of the conditions provided in the sections
above, the DONOR or its assigns, successors-interest, may take such action,
including the revocation of the donation, in order to protect its interest.
77. Upon the revocation of the donation, the DONEE shall forthwith return to the
DONOR within 30 days the donated items above enumerated without prejudice
to damages.
78. This Deed of Donation is executed to effect the agreement of the parties and the
conveyance in a Memorandum of Agreement between the DONOR and
DONEE dated ________________ herewith attached. In case of non-compliance
or violation of the Agreement by herein DONEE, the facilities and items
conveyed shall revert back to the DONOR.
79. The DONEE shall commit to the sustained operations of the iSchools Learning
Resource Center and shall use the computer hardware units, network
equipment, and electronic peripherals donated by the DONOR exclusively for
the operations of the iSchools LRC.
80. The parties hereto mutually agree that this donation in favor of the DONEE
does not in any way bind the DONOR to grant future requests for donation by
the DONEE.

NOW THEREFORE, for and in consideration of the performance and observance of the
above obligations, the said DONOR hereby cedes, transfers and conveys by way of
Donation, unto said DONEE the personal property above-described free from all liens
and encumbrances.

ACCEPTANCE

The DONEE hereby ACCEPTS the foregoing donation of the above-described

property for with its sincerest appreciation and gratitude for the kindness and
liberality of the DONOR.

In witness whereof, the parties hereto have set their hands at the place first
